妇幼专科医院中药饮片精细化管理制度的建立与实施效果评价

摘　　要：目的分析医院中药饮片在贮存养护过程中出现的问题,建立中药饮片贮存养护精细化管理制度,规范和提高中药房中药饮片管理质量。方法整理2020年11月~2022年10月验收合格的7536批次中药饮片,共258次养护管理和质量控制的数据,分为对照组和观察组各129次。对照组采用常规管理方式,观察组采用精细化管理方式。对比两种贮存养护管理方式下中药饮片变异率、变异品种数、不合格中药饮片检出率、中药饮片养护及质量管理结果,分析中药房中药饮片在贮存养护过程中发生变异的原因及存在问题。结果观察组变异率低于对照组(0.49%vs.1.07%)(χ^(2)=9.037,P=0.003)、变异中药饮片品种数少于对照组(χ^(2)=4.957,P=0.026),不合格中药饮片检出率低于对照组(χ^(2)=6.159,P=0.013),不符合中药饮片储存保管要求检出次数低于对照组(χ^(2)=6.663,P=0.010)。结论精细化管理方式能显著减少中药饮片贮存过程中质量变异情况的发生,有效提高中药饮片贮存质量和减少中药资源浪费,为临床安全用药及临床疗效做好基础保障。Objective To analyze the problems during the storage and maintenance of Chinese herbal decoction pieces in hospitals,establish a refined management system for storage and maintenance,and standardize and improve the management quality of Chinese herbal decoction pieces in Chinese herbal pharmacies.Methods A total of 258 instances of maintenance management and quality control data of 7536 batches of qualified Chinese herbal decoction pieces that passed acceptance inspection from November 2020 to October 2022 were sorted out and divided into the control group and the observation group,129 instances in each group.The control group adopted routine management mode,and the observation group adopted refine management method.The variation rate,number of variation varieties,detection rate of unqualified Chinese herbal decoction pieces,and maintenance and quality management results of the two storage and maintenance management methods were compared,and the causes of variation and existing problems during the storage and maintenance of Chinese herbal decoction pieces in pharmacy were analyzed.Results The variation rate of the observation group was lower than that of the control group(0.49%vs.1.07%)(χ2=9.037,P=0.003).The number of varieties of Chinese herbal decoction pieces in the observation group was fewer than that of the control group(χ2=4.957,P=0.026).The detection rate of unqualified Chinese herbal decoction pieces was lower than that of the control group(χ2=6.159,P=0.013).The number of instances that did not meet the storage and storage requirements for Chinese herbal decoction pieces was lower than that of the control group(χ2=6.663,P=0.010).Conclusion The implementation of refined management can significantly reduce the occurrence of quality variation during the storage of Chinese herbal decoction pieces,effectively improve the storage quality,and reduce the waste of Chinese herbal resources,and provide basic guarantee for clinical safe medication and clinical efficacy.
