温州地区汉族人群氯吡格雷药物代谢酶CYP2C19基因多态性分析

摘  要:目的分析温州地区汉族人群氯吡格雷药物代谢酶CYP2C19基因多态性分布特点。方法选取2022年1月—2023年7月进行过CYP2C19基因检测的温州地区普通汉族人群431例,分析该人群CYP2C19基因多态性,并与国内已经报道的其他地区汉族人群数据进行对比分析。结果温州地区汉族人群CYP2C19^(*)1、^(*)2、^(*)3等位基因的分布频率分别为60.0%、35.9%、4.1%,CYP2C19^(*)1/^(*)1、CYP2C19^(*)1/^(*)2、CYP2C19^(*)1/^(*)3、CYP2C19^(*)2/^(*)2、CYP2C19^(*)2/^(*)3、CYP2C19^(*)3/^(*)3基因型的分布频率分别为35.0%(151/431)、44.3%(191/431)、5.8%(25/431)、12.8%(55/431)、1.9%(8/431)、0.2%(1/431),CYP2C19快代谢型、CYP2C19中间代谢型、CYP2C19慢代谢型的分布频率分别为35.0%、50.1%和14.9%。≤60岁和>60岁温州地区汉族人群之间CYP2C19各等位基因、基因型、代谢型的分布无显著差异(P均>0.05)。男性与女性温州地区汉族人群之间CYP2C19各等位基因、基因型、代谢型的分布无显著差异(P均>0.05)。温州地区汉族人群CYP2C19各等位基因、基因型、代谢型分布与台州、湖州、湖北、金华、云南、宁德、苏州、沈阳、重庆、昆明、新疆地区汉族人群比较均无显著差异(P均>0.05),但与四川、杭州地区汉族人群比较均有统计学差异(P均<0.05);温州地区汉族人群CYP2C19各等位基因分布与广州地区汉族人群比较均有统计学差异(P均<0.05)。结论温州地区汉族人群中CYP2C19基因多态性分布不存在年龄、性别差异,与我国大多数地区的汉族人群相似,但与我国少数地区的汉族人群存在差异。Objective To analyze the distribution characteristic of Clopidogrel-metabolizing enzyme CYP2C19 gene polymorphphisms in Han population of Wenzhou region.Methods A total of 431 Han population in Wenzhou who were tested for CYP2C19 gene between January 2022 to July 2023 were enrolled.The CYP2C19 gene polymorphisms were ana-lyzed,and were compared with the existing data of other areas in China.Results The frequencies of CYP2C19^(*)1,^(*)2,^(*)3 alleles were 60%,35.9%,and 4.1%,respectively.The frequencies of CYP2C19^(*)1/^(*)1,^(*)1/^(*)2,^(*)1/^(*)3,^(*)2/^(*)2,^(*)2/^(*)3,^(*)3/^(*)3 genotypes were 35%(151/431),44.3%(191/431),5.8%(25/431),12.8%(55/431),1.9%(8/431),0.2%(1/431),respectively.The frequencies of extensive metabolizers,intermediate metabolizers and poor metabolizers were 35%(151/431),50.1%(216/431),and 14.9%(64/431),respectively.There were no significant differences in the distribution of CYP2C19 alleles,genotypes,or metabolizers between individuals aged≤60 years and>60 years in the Han Chinese population of Wenzhou region(all P>0.05).There were no significant differences in the distribution of CYP2C19 alleles,genotypes or metabolizers between male and females(all P>0.05).The distributions of CYP2C19 al-leles,genotypes and metabolizers in Wenzhou Han population were not significantly different from those in Taizhou,Hu-zhou,Hubei,Jinhua,Yunnan,Ningde,Suzhou,Shenyang,Chongqing,Kunming and Xinjiang Han populations(all P>0.05).However,the distributions were significantly different from those of Han population in Sichuan and Hangzhou(all P<0.05).There were statistically significant differences in the distribution of all CYP2C19 alleles between the Han Chi-nese populations in Wenzhou and Guangzhou regions.Conclusion The distribution of CYPC219 gene polymorphism in the Han population in Wenzhou region not related to age or gender,and is similar to that of Han population in most areas of China,but it is different from that of a few regions of China.
