摘  要:近年来,中医药治疗少弱精子性不育症的基础研究被广泛展开。磷脂肌醇3-激酶(phosphoinositide-3 kinase,PI3K)/蛋白激酶B(protein kinase B,AKT)信号通路作为调控细胞生长、增殖和凋亡的关键通路,在精子发生和成熟过程中具有重要作用。菟丝子黄酮、淫羊藿苷等中药单体,黄精、仙茅等单味中药,菟丝子-枸杞子、牛膝-王不留行等经典药对,以及五子衍宗丸、左归丸、益肾强脾方等复方制剂,均可通过调控该通路促进生精细胞增殖、抑制生精细胞凋亡及调节氧化应激等治疗少弱精子性不育症。现有研究仍存在一定局限性,如主要集中于基础实验,缺乏大样本随机对照临床试验,中医外治法通过PI3K/AKT信号通路治疗该病的研究尚属空白,今后的研究应着重于开展规范化临床研究,探索多元化治疗方案,充分发挥中医药特色优势,为少弱精子性不育症的临床治疗提供新思路。In recent years,the basic research of traditional Chinese medicine in the treatment of oligospermia has been widely carried out.As a key pathway regulating cell growth,proliferation and apoptosis,PI3K/AKT signaling pathway plays an important role in spermatogenesis and maturation.Traditional Chinese medicine monomers such as dodder flavonoids and icariin,single Chinese medicines such as Huangjing(Rhizoma polygonati)and Xianmao(Rhizoma curculiginis),classic drug pairs such as Tusizi-Gouqizi(Semen Cuscutae-Lycium berry)and Niuxi-Wangbuliuxing(Achyranthes bidentata-Cowherb seed)as well as compound preparations such as Wuzi Yanzong Pill,Zuogui Pill and Yishen Qiangpi Formula,can promote the proliferation of spermatogenic cells,inhibit the apoptosis of spermatogenic cells,and regulate oxidative stress to treat oligospermic infertility by regulating the pathway.There are still some limitations in the existing research,such as mainly focusing on basic experiments,lack of large-sample randomized controlled clinical trials,and the research on the treatment of this disease through PI3K/AKT signaling pathway by external treatment of traditional Chinese medicine is still blank,and future research should focus on carrying out standardized clinical research,exploring diversified treatment options,giving full play to the advantages of traditional Chinese medicine,and providing new ideas for the clinical treatment of oligoasthenospermia infertility.
