摘  要:精子DNA损伤是影响男性生育力的重要因素,对妊娠结局影响较大,其发生机制主要为精子生成异常、染色质异常、氧化应激反应等。现代医学以抗氧化剂治疗、微循环治疗及辅助生殖技术为主,治疗手段较为单一,且标准化不足。中医药治疗该病以补肾活血、清热利湿、补肾理肝等为主,存在治疗周期及疗效判定标准不一、缺乏循证医学证据、集中于临床观察、动物实验匮乏等问题。未来的研究应深入探讨精子DNA损伤的发病机制,建立规范的循证医学研究体系,加强动物和细胞实验,广泛开展治疗机制研究,为临床诊断和治疗男性不育提供新思路。Sperm DNA damage is an important factor affecting male fertility and has a great impact on pregnancy outcomes,and its mechanism is mainly abnormal spermatogenesis,chromatin abnormalities,oxidative stress response,etc.Modern medicine mainly focuses on antioxidant therapy,microcirculation therapy and assisted reproductive technology,but the treatment methods are relatively simple and lack of standardization.Traditional Chinese medicine(TCM)treatment is mainly based on tonifying the kidney and invigorating the blood,clearing away heat and dampness,tonifying the kidney and regulating the liver,etc.,and there are some problems,namely,different treatment cycles and efficacy judgment criteria,a lack of evidence-based medical evidence,focusing on clinical observation,and a lack of animal experiments.In future researches,the pathogenesis of sperm DNA damage should be deeply explored,to establish a standardized evidence-based medical research system,to strengthen animal and cell experiments,and to carry out extensive research on the treatment mechanism,so as to provide new ideas for clinical diagnosis and treatment of male infertility.
