基于“休眠激活”理论从阴阳论治腰椎间盘突出

摘　　要：基于阴阳学说理论以及腰椎间盘突出在现代解剖、生理、病理医学中的本质联系,提出“休眠激活理论”:患肌因疼痛等因素引起被动功能减退以缓冲损伤(阳衰),其协同肌则代偿性超负荷运作,以填补患肌功能的空缺;机体通过建立此种短暂的“病态平衡”,导致患肌功能每况愈下,协同肌愈发疲劳;日久引发肌肉、神经乃至骨骼的功能显著受限,伴随水肿、炎性渗出的增加(阴盛),发为此病。本病属“阴盛阳虚”范畴,可以针刺打破“病态平衡”,主张“扶阳抑阴”为治法,选穴气海俞,气海俞旁开1.5寸阿是穴,第4、5腰椎华佗夹脊穴、腰眼、坤穴以恢复腰背阴阳。Based on the theory of Yin-Yang as well as the essential connection of that with lumbar intervertebral disc herniation in modern anatomical,physiological and pathological medicine,the"dormant activation theory"is proposed:the affected muscle suffers from passive function loss due to pain and other factors in order to cushion the damage(Yang deficiency),and the synergistic muscle operates with compensatory overload in order to fill the vacancies of the affected muscle′s function.By establishing such a transient"pathological balance",the function of the affected muscle deteriorates,and the synergistic muscle becomes more and more fatigued;over time,this triggers a significant limitation of the function of the muscles,nerves,and even the bones,along with an increase in oedema and inflammatory exudation(Yin excess),which leads to the development of this disease.This disease belongs to the category of"Yin excess and Yang deficiency"and can be broken by acupuncture to break the"pathological equilibrium".Therefore,it is advocated that the treatment of"supporting Yang and suppressing Yin",and Qihai Shu acupoint,Ashi acupoint1.5 Cun from the Qihai Shu,Huatuo pincer spine acupoint on the 4th and 5th lumbar vertebrae,lumbar eye acupoint and Kun acupoint were chosen can be selected in treatment to restore the Yin and Yang balance of the back.
