中药调控PI3K/AKT信号通路防治胃癌研究进展

摘　　要：磷脂酰肌醇3-激酶/蛋白激酶B(phosphoinositide-3 kinase/protein kinase B,PI3K/AKT)信号通路在胃癌的发生发展中发挥关键作用,而中药可通过干预PI3K/AKT下游mTOR/Sox2靶标、SCL7A11(xCT)介导的铁死亡、转化生长因子-β(transforming growth factor-β,TGF-β)/Smad信号通路、PI3K/AKT/缺氧诱导因子-1(hypoxia inducible factor-1,HIF-1)、Bax/Bcl-2线粒体凋亡途径、核转录因子-κB(nuclear transcription factor-κB,NF-κB)信号通路等,调控胃癌细胞的增殖、自噬、分化、凋亡、侵袭等细胞过程。中药可基于PI3K/AKT信号通路预防胃癌前病变,在阻断炎-癌转化、抑制胃癌肝转移方面独具优势,但现有研究仍存在临床转化有待加强、作用机制研究有待深入、缺乏毒理机制与安全效应评价研究等不足,未来需进一步开展临床观察,阐明中药活性成分通过该通路防治胃癌的具体机制,研发新型中药制剂,同时在中医理论指导下,探讨PI3K/AKT及其相关通路与不同胃癌证型的对应关系,加强多学科交叉及大样本研究,构建“中药-成分-靶点-通路-证型-胃癌”网络体系,为胃癌的防治提供新的研究思路。The phosphoinositide-3 kinase/protein kinase B(PI3K/AKT)signaling pathway plays a key role in the occurrence and development of gastric cancer,and traditional Chinese medicine can intervene in the downstream mTOR/Sox2 target,SCL7A11(xCT)-mediated ferroptosis,and transforming growth factor(β)by intervening in the downstream of PI3K/AKT Factor-β,TGF-β)/Smad signaling pathway,PI3K/AKT/hypoxia inducible factor-1(HIF-1),Bax/Bcl-2 mitochondrial apoptosis pathway,nuclear transcription factor-κB(NF-κB)signaling pathway,etc.,regulate the proliferation,autophagy,differentiation,apoptosis,and apoptosis of gastric cancer cells.invasion and other cellular processes.Traditional Chinese medicine can prevent gastric precancerous lesions based on the PI3K/AKT signaling pathway,and has unique advantages in blocking inflammation-cancer transformation and inhibiting liver metastasis of gastric cancer,but the existing research still has shortcomings,namely,the clinical transformation to be strengthened,mechanism of action to be deepened,a lack of toxicological mechanism and safety effect evaluation research,etc.Therefore,further clinical observation is needed in the future,to elucidate the specific mechanism of the prevention and treatment of gastric cancer through the pathway of traditional Chinese medicine active ingredients,to develop new traditional Chinese medicine preparations,and to explore the correspondence between PI3K/AKT and its related pathways and different gastric cancer syndrome types.Besides,multidisciplinary and large-sample studies should be strengthened,and a network system of"traditional Chinese medicine-component-target-pathway-syndrome-gastric cancer"should be constructed,to provide new research ideas for the prevention and treatment of gastric cancer.
