升阳益胃汤联合雷贝拉唑治疗慢性萎缩性胃炎临床观察

摘　　要：目的探讨升阳益胃汤联合雷贝拉唑治疗慢性萎缩性胃炎(CAG)的临床疗效。方法选取医院2021年6月至2023年12月收治的CAG患者120例,按随机数字表法分为对照组和研究组,各60例。两组患者均予雷贝拉唑钠肠溶片,研究组患者加用升阳益胃汤。两组患者均连续治疗4周。结果研究组总有效率为93.33%,显著高于对照组的80.00%(P<0.05)。治疗后,两组患者的胃黏膜组织学指标(胃窦黏膜萎缩、肠上皮化生、炎性反应、病变活动度)积分,中医证候(胃脘疼痛、倦怠乏力、食少纳呆、口苦)积分均显著低于治疗前(P<0.05),且研究组均显著低于对照组(P<0.05);两组患者胃泌素17(G17)、胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)水平和PGR(PGⅠ/PGⅡ)均显著高于治疗前(P<0.05),且研究组均显著高于对照组(P<0.05)。研究组和对照组患者不良反应发生率相当(18.33%比21.67%,P>0.05)。结论升阳益胃汤联合雷贝拉唑治疗CAG的临床疗效良好,可改善患者的胃黏膜组织学指标和中医证候,调节胃肠激素水平,且安全性良好。Objective To investigate the clinical efficacy of Shengyang Yiwei Decoction combined with rabeprazole in the treatment of chronic atrophic gastritis(CAG).Methods A total of 120 patients with CAG admitted to the hospital from June 2021 to December 2023 were selected and randomly divided into the control group and the study group by the random number table method,with 60 cases in each group.The patients in the two groups were treated with Rabeprazole Sodium Enteric-Coated Tablets,while the patients in the study group were treated with Shengyang Yiwei Decoction.Both groups were treated continuously for four weeks.Results The total effective rate in the study group was 93.33%,which was significantly higher than 80.00%in the control group(P<0.05).After treatment,the scores of gastric mucosal histological indexes(atrophy of gastric antral mucosa,intestinal metaplasia,inflammatory reaction,and lesion activity)and traditional Chinese medicine(TCM)syndrome scores(epigastric pain,fatigue,poor appetite,and bitter mouth)in the two groups were significantly lower than those before treatment(P<0.05),and those in the study group were significantly lower than those in the control group(P<0.05);the levels of gastrin 17(G17),pepsinogenⅠ(PGⅠ),pepsinogenⅡ(PGⅡ),and PGR(PGⅠ/PGⅡ)in the two groups were significantly higher than those before treatment(P<0.05),and those in the study group were significantly higher than those in the control group(P<0.05).The incidence of adverse reactions was comparable between the study group and control group(18.33%vs.21.67%,P>0.05).Conclusion Shengyang Yiwei Decoction combined with rabeprazole has good clinical efficacy and safety in the treatment of CAG,which can improve the patients'histological indexes of gastric mucosa and TCM syndromes,and regulate gastrointestinal hormone levels.
