射麻口服液联合吸入用布地奈德混悬液对肺炎支原体肺炎患儿免疫功能与外周血Toll样受体2信号通路表达水平的影响

摘　　要：目的探讨射麻口服液联合吸入用布地奈德混悬液对肺炎支原体肺炎患儿免疫功能与外周血Toll样受体2(TLR2)信号通路表达水平的影响。方法选取安徽省安庆市岳西县中医院2022年1月至2023年12月收治的肺炎支原体肺炎患儿104例,按随机数字表法分为观察组和对照组,各52例。两组患儿均予常规治疗和吸入用布地奈德混悬液,观察组患儿加用射麻口服液,两组患儿均连续治疗7 d。结果治疗后,两组患儿的中医证候积分、C反应蛋白、降钙素原、白细胞介素6、TLR2、髓样分化因子88(MyD88)、核转录因子-κB(NF-κB)表达水平及TLR2,MyD88,NF-κB mRNA表达量均显著降低(P<0.05),且观察组均显著低于对照组(P<0.05);两组患儿的免疫球蛋白A、免疫球蛋白G、免疫球蛋白M表达水平均显著升高(P<0.05),且观察组均显著高于对照组(P<0.05)。观察组的临床总有效率为96.15%,显著高于对照组的80.77%(P<0.05);观察组和对照组的不良反应发生率相当(13.46%比9.62%,P>0.05)。结论射麻口服液联合吸入用布地奈德混悬液治疗肺炎支原体肺炎的临床疗效良好,可改善患儿的临床症状,增强免疫功能,且安全性较好。其可能通过下调TLR2信号通路的表达,以减轻下游炎性反应的发生。Objective To investigate the effect of Shema Oral Liquid combined with Suspension for Inhalation on immune function and Toll-like receptor 2(TLR2)signaling pathway in the peripheral blood of children with Mycoplasma pneumoniae pneumonia.Methods A total of 104 children with Mycoplasma pneumoniae pneumonia admitted to the Yuexi Hospital of Traditional Chinese Medicine from January 2022 to December 2023 were selected and divided into the observation group and the control group by the random number table method,with 52 cases in each group.The children in the two groups were treated with conventional therapy and Budesonide Suspension for Inhalation,while the patients in the observation group were treated with Shema Oral Liquid.Both groups were treated continuously for 7 d.Results After treatment,the traditional Chinese medicine(TCM)syndrome score,C-reactive protein,procalcitonin,interleukin-6,the expression levels of TLR2,myeloid differentiation factor 88(MyD88),and nuclear transcription factor-κB(NF-κB),and the expression levels of TLR2,MyD88,NF-κB mRNA in the two groups significantly decreased(P<0.05),and those in the observation group were significantly lower than those in the control group(P<0.05).The expression levels of immunoglobulin A,immunoglobulin G,and immunoglobulin M in the two groups significantly increased(P<0.05),and those in the observation group were significantly higher than those in the control group(P<0.05).The total clinical effective rate in the observation group was 96.15%,which was significantly higher than 80.77%in the control group(P<0.05).The incidence of adverse reactions was comparable between the observation group and the control group(13.46%vs.9.62%,P>0.05).Conclusion Shema Oral Liquid combined with Suspension for Inhalation has good clinical efficacy and safety in the treatment of Mycoplasma pneumoniae pneumonia,which can improve the clinical symptoms of children,and enhance immune function.Its mechanism may be reducing the occurrence of downstream inflammatory reactions by down-reg
