独活寄生汤治疗维持性血液透析继发不宁腿综合征临床观察

摘　　要：目的观察独活寄生汤熏洗治疗肝肾不足、气血亏虚型维持性血液透析(MHD)继发不宁腿综合征(RLS)的临床效果。方法将上犹县中医院肝肾不足、气血亏虚型MHD继发RLS患者64例作为研究对象,分为对照组和观察组,各32例。对照组予规律血液透析,观察组在此基础上联合独活寄生汤熏洗治疗,观察两组临床疗效及治疗前后中医证候积分,实验室检查指标水平,不宁腿综合征评定量表(RLSRS)、匹兹堡睡眠质量指数量表(PSQI)、汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)评分及不良事件发生率。结果观察组治疗总有效率为81.25%(26/32),高于对照组的53.12%(17/32)(P<0.05)。治疗后,观察组RLSRS、PSQI、HAMD、HAMA评分降低、实验室检查指标水平改善且均优于对照组(P<0.05)。两组均未出现明显不良反应。结论独活寄生汤熏洗治疗肝肾不足、气血亏虚型MHD继发RLS,临床效果显著,可改善患者焦虑,提高其生活质量且安全性良好。Objective To observe the clinical effect of Duhuo Jisheng decoction fumigation and washing in the treatment of restless leg syndrome(RLS)secondary to maintenance hemodialysis(MHD)of deficiency of liver and kidney and deficiency of qi and blood type.Methods A total of 64 patients with RLS secondary to MHD with deficiency of liver and kidney and deficiency of qi and blood type in Shangyou County Hospital of Traditional Chinese Medicine were divided into control group and observation group,with 32 cases in each group.The control group was given regular hemodialysis,and the observation group was combined with Duhuo Jisheng decoction fumigation and washing treatment on this basis.The clinical efficacy,TCM syndrome scores,laboratory index levels,Restless leg Syndrome Rating Scale(RLSRS),Pittsburgh Sleep Quality Index(PSQI),Hamilton Depression Scale(HAMD),Hamilton Anxiety Scale(HAMA)scores and the incidence of adverse events were observed in the two groups before and after treatment.Results The total effective rate of the observation group was 81.25%(26/32),which was higher than that of the control group(53.12%,17/32)(P<0.05).After treatment,the scores of RLSRS,PSQI,HAMD and HAMA decreased,and the level of laboratory examination indexes improved,which were all better than those in the control group(P<0.05).There were no obvious adverse reactions in both groups.Conclusion Duhuo Jisheng decoction fumigation and washing has a significant effect in the treatment of RLS secondary to MHD of deficiency of liver and kidney and deficiency of qi and blood type,improves anxiety,improves quality of life,and has good safety.
