摘  要:目的分析重型颅脑损伤患者血清可溶性髓系细胞触发受体-1(sTREM-1)、降钙素基因相关肽(CGRP)、高迁移率族蛋白B1(HMGB1)和胆碱酯酶(ChE)水平与并发肺部感染的相关性。方法选择2021年12月-2023年12月本院85例重型颅脑损伤患者作为研究对象,按是否并发肺部感染分为感染组(35例)和对照组(50例)两组。感染组患者又根据临床肺部感染评分(CPIS)分为轻度组(CPIS≤6分,25例)和重度组(CPIS>6分,10例)两组。比较两组患者血清sTREM-1、CGRP、HMGB1和ChE水平差异。分析血清sTREM-1、CGRP、HMGB1和ChE水平与CPIS评分的相关性。Logistic回归分析筛选影响重型颅脑损伤患者并发肺部感染的独立危险因素。ROC曲线分析sTREM-1、CGRP、HMGB1和ChE水平对重型颅脑损伤患者并发肺部感染的诊断效能。结果感染组患者血清sTREM-1、HMGB1水平均显著高于对照组,CGRP、ChE水平均显著低于对照组,差异具有统计学意义(P<0.05);CPIS评分与sTREM-1和HMGB1水平呈显著正相关(P<0.05),而与CGRP和ChE水平呈显著负相关(P<0.05);单因素分析结果显示,血清sTREM-1、CGRP、HMGB1和ChE水平是患者是否并发肺炎的影响因素(P<0.05);多因素Logistic回归分析结果显示,患者血清sTREM-1、HMGB1水平升高、CGRP以及ChE水平降低均是影响重型颅脑损伤患者并发肺炎的独立危险因素(P<0.05);ROC曲线分析显示,血清sTREM-1、CGRP、HMGB1和ChE水平预测重型颅脑损伤患者并发肺部感染的AUC分别为0.9246、08366、0.8960、0.8354,具有临床预测价值。结论重型颅脑损伤患者血清sTREM-1、CGRP、HMGB1和ChE水平与患者肺部感染的发生发展密切相关,能为临床重型颅脑损伤患者肺部感染的早期诊断及病情评估提供重要依据。Objective To analyze the correlation between serum soluble triggering receptor expressed on myeloid cells-1(sTREM-1),calcitonin gene-related peptide(CGRP),high mobility group box 1 protein(HMGB1),and cholinesterase(ChE)levels and the occurrence of pulmonary infections in patients with severe traumatic brain injury.Methods a total of 85 patients with severe traumatic brain injury admitted to our hospital from december 2021 to december 2023 were selected.patients were divided into infection group(n=35)and control group(n=50)based on the presence of pulmonary infection.According to the clinical pulmonary infection score(CPIS)the patients in the infection group were divided into mild group(CPIS less than or equal to 6,n=25)and severe group(CPIS over 6,n=10).Serum levels of sTREM-1,CGRP,HMGB1,and ChE were compared between the two groups.The correlation between serum sTREM-1,CGRP,HMGB1,and ChE levels and CPIS scores was analyzed.Logistic regression was used to identify factors influencing the occurrence of pulmonary infections in these patients.The diagnostic efficacy of sTREM-1,CGRP,HMGB1,and ChE levels for pulmonary infections was evaluated using ROC curve analysis.Results Serum sTREM-1 and HMGB1 levels were significantly higher in the infection group than in the control group,while CGRPand ChE levels were significantly lower(P<0.05).CPIS scores were positively correlated with sTREM-1 and HMGB1 levels,and negatively correlated with CGRPand ChE levels(P<0.05).univariate analysis showed that serum sTREM-1,CGRP,HMGB1,and ChE levels were influencing factors for pulmonary infections(P<0.05).multivariate logistic regression analysis confirmed that elevated sTREM-1 and HMGB1 levels,and reduced CGRPand ChE levels were independent risk factors for pulmonary infections(P<0.05).roc curve analysis showed that the auc values for predicting pulmonary infections were 0.9246 for sTREM-1,0.8366 for CGRP,0.8960 for HMGB1,and 0.8354 for ChE,indicating clinical predictive value.Conclusions Serum levels of sTREM-1,CGRP,HMGB1,and ChE in p
