摘　　要：目的探讨血清25-羟基维生素D[25-(OH)D]、同型半胱氨酸(Hcy)与子痫前期(PE)患者病情严重程度的关系。方法选择2022年4月-2024年4月本院确诊的110例早发型PE患者纳入病例组,并根据病情程度将病例组分为轻度组(62例)和重度组(48例),另选取同期于医院产检的110名健康孕妇纳入对照组。比较血清25(OH)D、Hcy在各组间的表达差异,采用Spearman分析血清25(OH)D、Hcy水平与PE患者病情程度的关系,并分析两指标评估重度PE的价值。结果与对照组比较,病例组血清25(OH)D水平降低,Hcy水平升高(P<0.05);与轻度组比较,重度组收缩压、舒张压、24 h尿蛋白、血清Hcy水平均显著升高,血清25(OH)D水平明显降低(P<0.05);根据Spearman相关分析显示,PE患者病情程度与血清25(OH)D呈负相关(r=-0.424,P<0.05),与Hcy呈正相关(r=0.461,P<0.05);绘制受试者工作特征(ROC)曲线显示,血清25(OH)D、Hcy水平单独及联合评估重度PE的曲线下面积分别为0.747、0.768、0.842,联合评估的价值相对较高。结论血清25(OH)D、Hcy水平与PE患者病情严重程度,二者联合可提高临床评估重度PE的价值。Objective To investigate the relationship between serum 25-hydroxyvitamin D[25-(OH)D],homocysteine(Hcy)and the severity of preeclampsia(PE)in patients.Methods 110 early-onset PE patients diagnosed in the hospital from April 2022 to April 2024 were enrolled in the case group.This group was further divided into a mild group(62 cases)and a severe group(48 cases)based on the severity of the condition.Another 110 healthy pregnant women who underwent prenatal check-ups in the hospital during the same period were enrolled in the control group.The expression differences of serum 25(OH)D and Hcy among all groups were compared.The relationship between serum 25(OH)D and Hcy levels and the severity of PE patients was analyzed using Spearman correlation analysis.Additionally,the value of these two indexes in evaluating severe PE was analyzed.Results Compared with the control group,the serum 25(OH)D level decreased and the Hcy level increased in the case group(P<0.05).Compared with the mild group,the severe group showed significant increases in systolic blood pressure,diastolic blood pressure,24-hour urinary protein,and serum Hcy levels,while serum 25(OH)D levels were significantly reduced(P<0.05).According to Spearman correlation analysis,the severity of PE patients'condition was negatively correlated with serum 25(OH)D(r=-0.424,P<0.05)and positively correlated with Hcy(r=0.461,P<0.05).Receiver operating characteristic(ROC)curves showed that the areas under the curves for serum 25(OH)D and Hcy levels alone and in combination for assessing severe PE were 0.747,0.768,and 0.842,respectively,indicating that the value of combined assessment was relatively high.Conclusions The concentrations of serum 25(OH)D and Hcy are related to the severity of PE patients.The combination of serum 25(OH)D and Hcy levels can improve the clinical evaluation value of severe PE.
