育龄期系统性红斑狼疮女性患者生育忧虑心理体验的质性研究

摘　　要：1目的探讨育龄期系统性红斑狼疮(SLE)女性患者生育忧虑的心理体验,为制订针对性的干预措施提供依据。方法采用质性研究中的现象学研究方法,通过目的抽样法选取2024年4一5月在南方医科大学第三附属医院门诊就诊或住院的13例育龄期SLE女性患者进行半结构式访谈,使用Colaizzi7步分析法提炼主题。结果共访谈13例育龄期SLE女性患者,年龄为21~36(27.08±3.88)岁。共提炼出6个主题:对生育能力的忧虑、对自身健康的忧虑、对子代安全的忧虑、生育信息未满足的忧虑、正性情绪传递的需求、生育相关问题带来的负性心理体验(包括因难以维持亲密关系产生的自卑与沮丧,因生育压力产生的焦躁与愧疚,因生育失败经历产生的痛苦与恐慌,因生育成本负担产生的不安与焦虑)。结论育龄期SLE女性患者针对生育问题存在诸多忧虑,医护人员应重视育龄期SLE女性患者生育相关心理体验,针对性地给予科学、有效的干预,缓解患者生育有关的负性心理体验。Objective To explore the psychological experience of reproductive concerns in female patients of childbearing age with systemic lupus erythematosus(SLE),and to provide evidence for formulating targeted intervention measures.Methods From April to May 2024,phenomenological research methods of qualitative study and purposive sampling were used to conduct semi-structured interviews with 13 female SLE patients of childbearing age who were outpatient or hospitalized in the Third Affiliated Hospital of Southern Medical University.Colaizzi's seven-step method was used for data analysis.Results A total of 13 female patients of childbearing age with SLE were interviewed,aged 21-36(27.08±3.88)years.Six themes were identified:concerns about fertility,concerns about one's own health,concerns about the safety of the offspring,concerns about unmet fertility information,the need for positive emotional transmission,negative psychological experience caused by childbearing related problems(inferiority and depression caused by difficulty in maintaining intimate relationships,anxiety and guilt caused by reproductive pressure,pain and panic caused by failure reproductive experience,and restlessness and anxiety caused by the burden of reproductive costs).ConclusionsF Iemale patients of childbearing age with SLE have many concerns about fertility issues.Medical staff should pay attention to the psychological experience related to fertility among female patients of childbearing age with SLE,and give targeted scientific and effective intervention to alleviate the negative psychological experience of patients with reproductive.
