补肾活血汤对膝骨关节炎大鼠的改善作用及对巨噬细胞极化的调控

摘　　要：目的 探讨补肾活血汤对膝骨关节炎大鼠的改善作用及调控巨噬细胞极化的作用机制研究。方法 40只SD大鼠随机分为假手术组(Sham)、模型组(Model)、补肾活血汤组(BSHX)及补肾活血汤+JAK2/STAT3信号通路激动剂Colivelin组(BSHX+Colivelin)(n=10)。HE染色与番红O-固绿染色观察大鼠滑膜组织与软骨组织病理变化,并进行滑膜Krenn评分与软骨Mankin评分;ELISA检测大鼠血清中IL-1β、TNF-α、IL-4、IL-10的表达水平;免疫荧光技术检测大鼠滑膜中M1/M2型巨噬细胞极化情况;Western blot检测滑膜组织中p-JAK2/JAK2、p-STAT3/STAT3比率。结果 补肾活血汤干预后,大鼠滑膜与软骨病理损伤明显改善,Krenn与Mankin评分显著降低;血清IL-1β、TNF-α表达水平显著降低,IL-4、IL-10表达水平显著升高,滑膜组织M1型巨噬细胞标记物表达降低,而M2型巨噬细胞标记物表达升高;p-JAK2/JAK2、p-STAT3/STAT3比率显著降低。此外,Colivelin部分逆转了补肾活血汤对上述指标的调控作用。结论 补肾活血汤是通过抑制JAK2/STAT3信号通路,促进滑膜巨噬细胞向M2极化,改善大鼠膝骨关节炎症状。Objective To study the effect of Bushen Huoxue Decoction on knee osteoarthritis rats and the mechanism of regulating macrophage polarization.Methods Forty rats were randomly divided into sham group,Model group,BSHX group and BSHX+Colivelin group(n=10).Pathological changes of synovial tissue and cartilage were observed by HE staining and O⁃solid green staining,and synovial Krenn score and cartilage Mankin score were performed.The levels of IL⁃1β,TNF⁃α,IL⁃4 and IL⁃10 in serum of rats were detected by ELISA.The polarization of M1/M2 macrophages in the synovial membrane of rats was detected by immunofluorescence.The ratios of p⁃JAK2/JAK2 and p⁃STAT3/STAT3 in synovial tissue were detected by Western blot.Results After the intervention of Bushen Huoxue Decoction,the pathological injury of synovium and cartilage was improved obviously,and the scores of Krenn and Mankin were decreased significantly.The levels of IL⁃1βand TNF⁃αwere significantly decreased,the levels of IL⁃4 and IL⁃10 were significantly increased,the expression of M1⁃type macrophage markers was decreased,while the expression of M2⁃type macrophage markers was increased,the p⁃JAK2/JAK2 and p⁃STAT3/STAT3 ratios were decreased significantly.In addition,Colivelin partially reversed the regulatory effect of Bushen Huoxue Decoction on the above indexes.Conclusion Bushen Huoxue Decoction may promote the polarization of synovium macrophages to M2 by inhibiting JAK2/STAT3 signaling pathway,and improve the symptoms of knee osteoarthritis in rats.
