血脂代谢异常对全身麻醉患者术后认知功能的影响

摘　　要：目的探讨血脂代谢异常与全身麻醉患者术后认知功能的关系。方法收集2022年1月至2023年12月在西安市第五医院行择期手术的全身麻醉患者共122例,入院后收集患者的一般资料及血脂结果,将患者分为高脂血症组和正常血脂组,比较两组患者在术前1 d、术后7 d简易精神量表(MMSE)评分。结果术后7 d,高脂血症组MMSE评分低于正常血脂组(P<0.05),高脂血症组术后认知功能障碍(POCD)率高于正常血脂组(P<0.05),高脂血症组患者各个亚组术后POCD率经比较差异有统计学意义(P<0.05),其中高胆固醇血症组及混合型高脂血症组POCD率分别为26.32%和26.67%。结论血脂代谢异常可能参与了全身麻醉患者POCD的发生。Objective To analyze the relationship between lipid metabolism disorder and postoperative cognitive dysfunction(POCD)patients with general anesthesia.Methods A total of 122 cases of general anesthesia patients had selected operation at the Xi’an Fifth Hospital from January 2022 to December 2023.General information and laboratory results of lipids of patients were collected after admission.Patients were divided into hyperlipidemia group and normal lipids group according to their levels of lipids before operation.The two groups were evaluated 1 days before and 7 days after surgery by the mini-mental state examination(MMSE).Results The MMSE score of the hyperlipidemia group was lower than that of the normal lipids group at 7 days after surgery(P<0.05).The rate of POCD 7 days after surgery in the hyperlipidemia group was higher than that in the normal lipids group(P<0.05).There were statistically differences in the postoperative POCD rates among the subgroups of patients in the hyperlipidemia group(P<0.05),the POCD rates of hypercholesterolemia group(26.32%)and mixed hyperlipidemia group(26.67%).Conclusion Lipid metabolism disorder may be involved in the occurrence of POCD in patients after general anesthesia.
