摘 要:目的了解宁夏农村居民的门诊服务满意度及其影响因素,为提高宁夏农村居民的门诊服务的满意度及促进分级诊疗制度的发展提供参考依据。方法基于2022年6月开展的宁夏南部山区农村家庭的调查数据库,运用安德森模型分析影响宁夏农村居民的门诊服务满意度的主要因素。结果宁夏农村居民中仅有756人就诊过,门诊利用率为3.67%;宁夏农村居民中69.45%选择县域医疗机构,对就诊机构的满意度为77.38%;单因素分析结果显示,不同年龄、不同职业、离家最近的医疗点不同、是否健康体检、两周就诊次数不同是宁夏农村居民门诊服务满意度的影响因素;多因素logistic回归分析结果显示,离家最近的医疗点为乡卫生院/县医院的满意度更低(OR=0.427,95%CI:0.225~0.809),两周就诊次数>1次的满意度更低(OR=0.595,95%CI:0.356~0.994),两周就诊首诊医疗机构为县级医院及以上的满意度更低(OR=0.432,95%CI:0.209~0.891)(OR=0.437,95%CI:0.204~0.939)(OR=0.342,95%CI:0.139~0.838),进行健康体检的农村居民的满意度更高(OR=1.758,95%CI:1.204~2.658)。结论居民对卫生服务利用的满意度是衡量分级诊疗制度建设成效的一项重要指标,宁夏农村居民的门诊服务的满意度有待进一步提升。Objective To understand the satisfaction of outpatient service of rural residents in Ningxia and ts influencing factors,and to provide reference for improving the satisfaction of outpatient service of rural residents in Ningxia and promoting the development of hierarchical diagnosis and treatment system.Methods Based on the survey database of rural families in mountainous areas of southern Ningxia conducted in June 2022,the Anderson model was used to analyze the main factors affecting the satisfaction of outpatient services in Ningxia rural residents.Results Only 756 rural residents in Ningxia have received medical treatment,and the outpatient utilization rate was 3.67%.Among rural residents in Ningxia,69.45%chose county medical institutions,and their satisfaction with the medical institution was 77.38%.The results of the univariate analysis showed that,different ages,different occupations,different medical points nearest to home,whether they had a healthy physical examination,and the different number of medical visits in two weeks were the influencing factors of the satisfaction of outpatient service utilization of rural residents in Ningxia.The results of multivariate logistic regression showed that the nearest medical point to the home was a township county hospital with lower satisfaction(OR=0.427,95%CI:0.225-0.809),the satisfaction rate was lower for more than one disease visit in two weeks(OR=0.595,95%CI:0.356-0.994).The satisfaction of the county-level hospital or above was even lower(OR=0.432,95%CI:0.209-0.891)(OR=0.437,95%CI:0.204-0.939)(OR=0.342,95%CI:0.139-0.838).Rural residents undergoing a health examination had higher satisfaction(OR=1.758,95%CI:1.204-2.658).Conclusion Residents'satisfaction with the utilization of health services is an important indicator to measure the effectiveness of the construction of the hierarchical diagnosis and treatment system,and the satisfaction of outpatient services of rural residents in Ningxia needs to be further improved.
