摘　　要：目的调查慢性心力衰竭(Chronic Heart Failure,CHF)患者需求现状并分析其影响因素,为科学制订CHF患者延续性护理计划提供参考依据。方法采用汉化版心力衰竭需求评估问卷(Heart Failure Needs Assessment Questionnaire,HFNAQ)对两家“三甲”医院心内科出院后1个月复诊的600名CHF患者进行调查。结果600名CHF患者HFNAQ条目均分为(3.13±0.74)分,其中自我管理维度得分最高;不同心功能分级患者自我管理维度得分均最高,且分级越高,自我管理需求得分越高。多元线性回归分析显示,性别和年龄是CHF患者需求的影响因素(P<0.05)。结论CHF患者HFNAQ各个维度的需求均处于中高水平,应全面、及时、有效地评估患者的个性化需求并制订相应的延续性护理措施,以满足其需求。Objective To investigate the current demand of patients with chronic heart failure(CHF)and analyze influencing factors to provide reference for the scientific development of continuity of care plans for patients with CHF.Methods A Chinese version of the Heart Failure Needs Assessment Questionnaire(HFNAQ)was used to investigate 600 patients with CHF who underwent follow-up visits one month after discharge from the cardiology departments of two Grade-A hospitals.Results The equal score of HFNAQ items for 600 patients with CHF was(3.13±0.74)score,with the self-management dimension scoring the highest.Patients with different levels of heart function classification have the highest scores in the self-management dimension,and the higher the level,the higher the score of self-management needs.Multiple linear regression analysis showed that gender and age were the influencing factors of demand of patients with CHF(P<0.05).Conclusion The needs of patients with CHF in all dimensions of HFNAQ are at a medium to high level.It is necessary to comprehensively,timely,and effectively evaluate the personalized demand of patients and develop corresponding continuity of care measures to meet their needs.
