基于隐结构模型与频繁项集的中药治疗颈椎病用药规律探析

摘　　要：目的探讨中医治疗颈椎病的辨证用药规律,以期为临床治疗提供参考。方法计算机检索中国知网、万方等数据库发表的有关中医药治疗颈椎病相关文献,检索时间为2013年1月至2022年12月,使用隐结构模型和频繁项集数据挖掘方法,分析颈椎病的辨证用药规律。结果筛选后共纳入文献133篇,使用数据挖掘方法,得出文献共涉及175项症状,包括舌淡红、头晕、颈项强痛、苔薄白、四肢麻木、颈项强直等,建立颈椎病的隐结构模型。统计中药处方的药物,共涉及163味中药,其中高频药物包括葛根、川芎、桂枝、当归、甘草等;症状-中药频繁项集5项,包含头晕+舌淡红+白术+天麻等;症状-证型-中药频繁项集5项,例如颈项强痛+痛处固定+舌有紫斑+气滞血瘀+桃仁+红花等。结论中药汤剂内服治疗颈椎病多以葛根、川芎、桂枝为主要药物,根据临床情况辨证配伍药物,以期为临床治疗颈椎病提供参考。Objective To explore Chinese herbal medicine selection pattern of cervical spondylosis,in order to provide reference for clinical treatment of cervical spondylosis.Methods The literatures on the treatment of cervical spondylosis by traditional Chinese medicine were collected from CNKI,VIP and Wanfang Data,etc.The time limit for retrieval was from January 2013 to December 2022.The information of Chinese herbal medicine selection was extracted to establish medical database of cervical spondylosis.Using latent structure model and frequency item to analyze Chinese herbal medicine inheritance support system of cervical spondylosis.Results A total of 133 papers and 175 Symptoms were screened out,which includes light red tongue,dizziness,strong neck pain,thin white moss,limbs numb,stiff neck,etc.and the latent structure model of Cervical spondylosis symptoms was established.Statistics showed that 163 TCM prescriptions were involved,among which the high frequency drugs included pueraria,ligusticum chuanxiong,cassia twig,angelica sinensis,licorice,etc.5 items of symptom-Chinese medicine frequent item set were excavated,including dizziness+pale tongue redness+atractylodes+gastrodia elata,etc.Symptom-syndrome type-Chinese medicine frequent item set 5 items,such as strong neck pain+pain fixed+tongue purple spots+Qi stagnation blood stasis+peach kernel+safflower,etc.Conclusion Chinese herbal medicine for cervical spondylosis selected Gegen(Pueraria Lobata),Chuanxiong(Szechuan Lovage Rhizome),Guizhi(ramulus cinnamomi)as main Chinese herbals,and selected other herbals based on clinical diagnosis and treatment.In order to provide reference for the clinical treatment of cervical spondylosis.
