摘 要:目的采用孟德尔随机化(MR)研究方法分析吸烟、饮酒、肥胖与下腰痛(LBP)发生风险的关联。方法采用双样本MR方法,以吸烟、饮酒、体质量指数(BMI)、腰围、臀围和体脂率相关的SNPs作为工具变量,所有SNPs均来自欧洲人群全基因组关联研究,采用逆方差加权法(IVW)估计吸烟、饮酒、BMI、腰围、臀围和体脂率与下腰痛的因果关系。同时,使用MR-Egger、加权中位数、加权模型和简单模型进一步评估因果关系。使用敏感性分析和多效性检验来测试结果的稳健性。结果遗传预测的现阶段吸烟(IVW,OR=3.85,95%CI=1.88~7.89,P=0.0002)、饮酒(IVW,OR=1.34,95%CI=1.15~1.55,P=0.0002)、BMI[IVW,OR=1.33,95%置信区间(CI)=1.21~1.46,P=2.31×10^(-9)]、腰围(IVW,OR=1.34,95%CI=1.19~1.51,P=2.19×10^(-6))、臀围(IVW,OR=1.27,95%CI=1.16~1.39,P=1.90×10^(-7))和体脂率(IVW,OR=1.47,95%CI=1.29~1.69,P=1.30×10^(-8))与下腰痛的发生呈正相关。敏感性分析表明,结果是可靠的,没有检测到水平多效性。结论本研究提供的证据表明现阶段吸烟、饮酒、肥胖是下腰痛的危险因素。Objective To evaluate the association of smoking,alcohol consumption and obesity with the risk of low back pain(LBP)using a Mendelian randomization(MR)approach.Methods Two-sample Mendelian randomization method was used to estimate the causal relationship between smoking,alcohol consumption,body mass index(BMI),waist circumference,hip circumference and body fat percentage as instrumental variables.All SNPs were from the European population genome-wide association study.Inverse variance weighting(IVW)method was used to estimate the causal relationship between smoking,alcohol consumption,BMI,waist circumference,hip circumference and body fat percentage and low back pain.At the same time,causality was further assessed using MR-Egger,weighted median,weighted model and simple model.Sensitivity analysis and pleiotropy tests were used to test the robustness of the results.Results Current smoking(IVW,OR=3.85,95%CI=1.88~7.89,P=0.0002),alcohol consumption[IVW,OR=1.34,95%confidence interval(CI)=1.15-1.55,P=0.0002],BMI(IVW,OR=1.33,95%CI=1.21-1.46,P=2.31×10^(-9)),waist circumference(IVW,OR=1.34,95%CI=1.19-1.51,P=2.19×10^(-6)),hip circumference(IVW,OR=1.27,95%CI=1.16-1.39,P=1.90×10^(-7))and body fat percentage(IVW,OR=1.47,95%CI=1.29-1.69,P=1.30×10^(-8))predicted by genetics were positively correlated with low back pain.Conclusion This study provides evidence that current smoking,alcohol consumption and obesity are causal risk factors for low back pain.
