摘 要:目的探讨健脾疏肝消浊法治疗2型糖尿病大鼠的药效作用与相关机制研究。方法SD大鼠(SPF级)21只随机分为正常组、模型组、中药组,每组7只。大鼠选用链脲佐菌素(STZ)腹腔注射,配合高脂饮食喂养,建立2型糖尿病大鼠模型。正常组和模型组予以生理盐水灌胃治疗,中药组选用健脾疏肝消浊中药复方干预8周。随后记录大鼠体重和进食量,检测生化指标:空腹血糖值(FBG)、餐后2小时血糖(2h PG)、空腹血清胰岛素(FINS)、血清总胆固醇(TC)、甘油三酯(TG)水平;采用HE染色进行大鼠胰腺和肝脏的病理生理学观察;通过免疫组化和蛋白印迹法(WB)检测大鼠组织胰岛素受体底物-1(IRS-1)和葡萄糖转运蛋白受体-4(GLUT-4)的表达情况。结果中药复方可降低SD大鼠体重、TG和FBG水平,促进FINS的释放,降低胰岛素抵抗指数(HOMA-IR);与模型组相比,中药组大鼠的胰腺胰岛形态明显改善,胰岛细胞结构较完整,细胞空泡和坏死明显减少;中药组肝细胞索排列较为整齐,肝窦清晰可见,肝细胞肿大、空泡变性明显减少,未见炎细胞浸润和纤维组织增生;同时,中药组的GLUT-4和IRS-1的蛋白表达水平显著高于模型组。结论健脾疏肝消浊法可有效改善2型糖尿病大鼠糖脂代谢紊乱并改善胰岛素抵抗情况,该作用可能通过调节肝脏组织IRS-1和GLUT-4蛋白的表达来实现。Objective To explore the pharmacological effects and underlying mechanisms of the Liver-soothing,Spleen-nourishing and Turbidity-resolving Therapy(健脾疏肝消浊法,LSTT)in treating type 2 diabetic rats based on the theory of visceral qi transformation.Methods Twenty-one SD rats(SPF grade)were randomly divided into three groups:the normal group,model group,and Chinese herbal medicine(CHM)group,with 7 rats in each group.A type 2 diabetes mellitus(T2DM)rat model was established by intraperitoneal injection of streptozotocin(STZ)combined with a high-fat diet.The normal and model groups received normal saline via gavage,while the CHM group was treated with LSTT for 8 weeks.Body weight and food intake were recorded,and biochemical indicators were measured,including fasting blood glucose(FBG),2-hour postprandial blood glucose(2h PG),fasting serum insulin(FINS),total cholesterol(TC),and triglycerides(TG)levels.Pathophysiological changes of the pancreatic and hepatic tissues were observed using HE staining.The expression levels of IRS-1 and GLUT-4 in tissues were detected by immunohistochemistry and Western blot(WB).Results LSTT reduced body weight,TG,and FBG levels in SD rats,promoted FINS secretion,and decreased the homeostasis model assessment of insulin resistance(HOMA-IR).Compared to the model group,the CHM group showed significant amelioration in pancreatic islet morphology,with more intact islet cell structures,and reduced cell vacuolation and necrosis.Hepatocyte cords in the CHM group were more orderly arranged,hepatic sinusoids were clearly visible,hepatocyte swelling and vacuolar degeneration were markedly reduced,and no inflammatory cell infiltration or fibrotic proliferation was observed.Additionally,protein expression levels of GLUT-4 and IRS-1 in the CHM group were significantly higher than those in the model group.Conclusion LSTT can effectively ameliorate glucolipid metabolism disorders and improve insulin resistance in type 2 diabetic rats,potentially by regulating the expressions of IRS-1 and GLUT-4
