摘　　要：总结陈文伯教授从精瘀论治癃闭(良性前列腺增生)的临证经验。陈教授认为肾精为一身生命之本,主藏精,肾气通过精道施泄肾精以生育后代,良性前列腺增生的核心病机即为精道瘀阻不通,肝脾肾不足、痰湿、瘀血等皆可导致精瘀,主张以化解精瘀为基本治则,分型论治。肝脾肾不足之精瘀当补益肾中阴阳、脾气肝血,方用自拟补肝脾肾通精汤;痰湿阻滞精瘀当利水渗湿除痰,方用自拟渗湿通精汤;血瘀精瘀当活血化瘀、开窍通精,方用自拟化瘀通精汤。To summarize Professor Chen Wenbo's clinical experience in treating retention of urine(benign prostatic hyperplasia,BPH)based on the theory of essence stasis.Professor Chen believes that kidney essence is the foundation of life,and the kidney governs the storage of essence.Kidney qi discharges essence through the spermatic pathway to facilitate reproduction.The core pathogenesis of BPH lies in obstruction of the spermatic pathway.Factors such as the liver,spleen,and kidney deficiency,phlegm-dampness,and blood stasis can all lead to essence stasis.He advocates resolving essence stasis as the fundamental treatment principle,combined with syndrome differentiation and targeted therapy.The essence stasis caused by deficiency of the liver,spleen,and kidney,so the treatment should focus on tonifying both yin and yang of the kidney,strengthening the spleen qi,and nourishing the liver blood.The formula used is Buganpishen Tongjing Decoction(补肝脾肾通精汤).For essence stasis caused by phlegm-dampness obstruction,the treatment should promote water discharge,drain dampness,and resolve phlegm.The formula used is Shenshi Tongjing Decoction(渗湿通精汤).For essence stasis caused by blood stasis,the treatment should activate blood circulation,resolve stasis,open the orifices,and unblock essence.The formula used is Huayu Tongjing Decoction(化瘀通精汤).
