经直肠腔内超声与实时组织弹性成像鉴别直肠肿物的效果观察

摘  要:目的:分析经直肠腔内超声与实时组织弹性成像鉴别直肠肿物良恶性的效果。方法:选取2020年1月至2022年5月,本院收治的86例拟行手术治疗的直肠肿物患者,均行经直肠腔内超声及实时组织弹性成像检查,术后以病理学诊断结果为标准。对比分析直肠腔内超声与实时组织弹性成像鉴别肿物良恶性的效果。结果:经病理学诊断,本组86例患者中良性病变9例,恶性77例;直肠腔内超声诊断良性病变14例,恶性72例;实时组织弹性成像诊断良性病变19例,恶性67例。直肠腔内超声判断直肠肿物良恶性的灵敏度为92.21%(71/77)、准确度为91.86%(79/86)、特异度为88.89%(8/9);实时组织弹性成像判断直肠肿物良恶性的灵敏度为84.42%(65/77)、准确度为83.72%(72/86)、特异度为77.78%(7/9)。直肠腔内超声检查与实时组织弹性成像诊断结果的灵敏度、准确度和特异度比较,差异无统计学意义(P>0.05)。结论:实时组织弹性成像技术有助于收集直肠肿物的硬度信息,有利于为进一步诊治提供依据,但与经直肠腔内超声相比,并未提高直肠肿物良恶性鉴别的准确性。Objective To analyze the effect of transrectal endoluminal ultrasound and real-time tissue elastography in differentiating benign and malignant rectal masses.Methods From January 2020 to May 2022,total 86 patients with rectal masses who were to be treated by surgery in our hospital were selected.Transrectal endoluminal ultrasound and real-time tissue elastography were performed,and pathological diagnosis results were used as the standard after surgery.The effect of transrectal endoluminal ultrasound and real-time tissue elastography in differentiating benign and malignant masses was compared and analyzed.Results Among the 86 patients,9 cases were benign and 77 cases were malignant.Transrectal endoluminal ultrasound diagnosed 14 benign lesions and 72 malignant lesions.Real-time tissue elastography diagnosed 19 benign lesions and 67 malignant lesions.The sensitivity,accuracy and specificity of transrectal endoluminal ultrasound were 92.21%(71/77),91.86%(79/86)and 88.89%(8/9)to determine benign and malignant rectal masses.The sensitivity,accuracy and specificity of real-time tissue elastography were 84.42%(65/77),83.72%(72/86)and 77.78%(7/9)to identify benign and malignant rectal masses.There was no significant difference in the sensitivity,accuracy and specificity between transrectal endoluminal ultrasound and real-time tissue elastography(P>0.05).Conclusion Real-time tissue elastography is helpful to collect the hardness information of rectal masses and provide evidence for further diagnosis and treatment.However,compared with transrectal endoluminal ultrasound,it does not improve the accuracy of distinguishing benign and malignant rectal masses.
