基于数据挖掘分析中医药治疗肛周湿疹的用药规律

摘　　要：目的:基于数据挖掘分析肛周湿疹的中医临床用药组方规律。方法:以“肛周湿疹”为主题词,“中医”或“中药”为自由词,检索中国知网、维普全文数据库及万方数据知识服务平台,严格按照纳入标准,搜集近30年来治疗肛周湿疹的中药方剂,筛选文献,建立数据库,运用中医传承辅助平台V2.5进行药物四气五味、归经及主要功效等分析,并研究规律。结果:最终纳入59篇文献,涉及110味中药。进行深度探索发现,有14种中草药的使用频率>10%,其中苦参、黄柏、地肤子、蛇床子和白鲜皮的使用频次较高,排在前五。出现频次≥20的药对有13个,“苦参、黄柏”“苦参、地肤子”“苦参、蛇床子”“黄柏、地肤子”和“黄柏、蛇床子”这5个药对的使用频次排在前五。结论:临床治疗肛周湿疹多从肝、胃、肾、脾、心经入手,以清热利湿为主要治则,多采用清热解表、利水渗湿结合攻毒杀虫止痒、活血祛瘀等方法选取相应中药组方治疗。Objective To analyze the medication regularity of traditional Chinese medicine(TCM)in treating perianal eczema based on data mining.Methods Literature on Chinese formulas for perianal eczema over the past 30 years was retrieved from CNKI,VIP,and Wanfang databases,using"perianal eczema"as the subject term and"TCM"or"Chinese medicinal"as free terms.After strict inclusion criteria screening,a database of Chinese medicinals and formulas in treating perianal eczema was established.The TCM Inheritance Support Platform V2.5 was utilized to analyze the four properties,five flavors,meridian tropism,efficacy,and study the regularity.Results A total of 59 articles involving 110 Chinese medicinals were included.Total 14 herbs showed a usage frequency>10%,with Kushen(Sophorae Flavescentis Radix),Huangbai(Phellodendri Chinensis Cortex),Difuzi(Kochiae Fructus),Shechuangzi(Cnidii Fructus),and Baixianpi(Dictamni Cortex)ranking in the top five.Total 13 herbal pairs had a frequency≥20.The top five herbal pairs were Kushen+Huangbai,Kushen+Difuzi,Kushen+Shechuangzi,Huangbai+Difuzi,and Huangbai+Shechuangzi.Conclusion The clinical treatment for perianal eczema primarily starts with liver,stomach,kidney,spleen,and heart meridians,and takes clearing heat and draining dampness as the main treatment principles.Chinese medicinals and formulas are selected based on the methods of clearing heat and relieving exterior syndrome,promoting urination and draining dampness,removing toxin,killing parasites,relieving itching,and promoting blood circulation to remove stasis.
