内镜下黏膜切除术与高频电凝切除术治疗结肠息肉的疗效及应激反应观察

摘　　要：目的:探讨内镜下黏膜切除术(EMR)与高频电凝切除术治疗结肠息肉的疗效及对应激反应的影响。方法:选取2021年1月至2022年12月,本院收治的84例结肠息肉患者,按随机数字表法分为观察组和对照组,每组各42例。观察组采用EMR治疗,对照组行内镜下高频电凝切除术治疗。比较2组围术期指标、疗效和应激反应等情况。结果:观察组术中失血量和术后住院时间均低于对照组(P<0.05),2组手术操作时间比较,差异无统计学意义(P>0.05)。观察组总有效率为95.2%,高于对照组的71.4%(P<0.05);谷胱甘肽过氧化物酶水平低于对照组(P<0.05),五羟色胺水平高于对照组(P<0.05),并发症发生率和复发率均低于对照组(P<0.05)。结论:与内镜下高频电凝切除术相比,EMR治疗结肠息肉的疗效更佳,可减少术中失血,减轻术后应激反应,减少并发症,加快患者康复,降低复发率。Objective To investigate the efficacy of endoscopic mucosal resection(EMR)and high-frequency electrocoagulation resection in the treatment of colon polyps and their effect on stress response.Methods Total 84 patients with colon polyps admitted to our hospital from January 2021 to December 2022 were divided into the observation group and the control group according to random number table method,with 42 cases in each group.The observation group was treated with EMR,and the control group was given endoscopic high-frequency electrocoagulation.Perioperative indexes,efficacy and stress response were compared between the two groups.Results The intraoperative blood loss and postoperative hospital stay in the observation group were lower than those in the control group(P<0.05).There was no statistical significance in the operation time between the two groups(P>0.05).The total effective rate of the observation group was 95.2%,which was higher than that of the control group(71.4%)(P<0.05).The level of glutathione peroxidase was lower than that of the control group(P<0.05),the level of serotonin was higher than that of the control group(P<0.05),and the complication rate and recurrence rate were lower than those of the control group(P<0.05).Conclusion Compared with endoscopic high frequency electrocoagulation resection,EMR is more effective in the treatment of colon polyps,which can reduce intraoperative blood loss,relieve postoperative stress reaction,decrease complications,accelerate the recovery of patients and lower the recurrence rate.
