摘　　要：目的:分析产前超声提示孤立性透明隔缺失(Isolated absent septum pellucidum,iASP)的超声特点及临床预后。方法:回顾性分析2018年10月—2023年10月于郑州大学第三附属医院超声科经产前系统超声检查发现的55例iASP胎儿,分析其超声图像、MRI结果、遗传学检查及临床预后等。结果:55例iASP胎儿中20例为透明隔完全缺失,双侧侧脑室前角相通呈“飞镖征”;35例透明隔部分缺失,其中29例为双侧透明隔均部分缺失,6例为单侧透明隔部分缺失。产前超声检查结果与产前MRI检查结果符合率为89.8%,研究发现产前MRI可发现更多颅脑信息。本研究中产前遗传学检查结果均为阴性。iASP临床预后差异较大,大部分预后良好。结论:产前超声诊断iASP准确性高,MRI可提供更多颅脑信息;完全型iASP生后多有不同程度的预后异常,部分型iASP大部分预后良好。Objective:To analyze the ultrasound characteristics and clinical prognosis of isolated absence of septum pellucidum(iASP).Methods:From October 2018 to October 2023,55 fetuses with iASP were identified through prenatal systematic ultrasound examination in the ultrasound department of the Third Affiliated Hospital of Zhengzhou University.The ultrasound images,MRI results,genetic examination and clinical prognosis were analyzed.Results:In 55 cases of iASP fetuses,there were 20 cases with complete absence of septum pellucidum and“dart sign”in the anterior horn of bilateral lateral ventricles,and 35 cases with partial absence of septum pellucidum,including 29 cases with partial absence of septum pellucidum in both sides and 6 cases with partial absence of septum pellucidum in one side.The coincidence rate of prenatal ultrasound and prenatal MRI was 89.8%,and we found that prenatal MRI could find more brain information.The results of prenatal genetic examination in this study were all negative.The clinical prognosis of iASP is quite different,and most of them have good prognosis.Conclusion:Prenatal ultrasound is highly accurate in diagnosing iASP,and MRI can provide additional information about the brain;Complete iASP is associated with varying degrees of abnormal prognosis after birth,and most cases of the partial iASP has good prognosis.
