摘　　要：良性前列腺增生症在老年男性中较为常见,所产生的尿路不适,对患者的生活质量具有严重影响。彭煜教授临证30载,在该病的治疗方面积累丰富的经验,他认为脏腑气机失调所引发的水液代谢障碍,是本病尿路不适的核心病机所在,在此基础上产生的痰湿、瘀血稽存状态,是引发腺体增大、尿路梗阻的关键。治疗主张恢复脏腑气化,形成温肾化气、滋肾清气、健脾益气、疏肝理气、提壶揭盖和宁心调气六种基本方法,并重视配合化痰除瘀散结药物以缩小增生的腺体,综合辨治,收效显著。附典型医案以论证。Benign prostatic hyperplasia(BPH)is relatively common in elderly men.The resulting urinary tract discomfort has a significant impact on the patients’quality of life.Professor PENG Yu,with over 30 years of clinical experience,has accumulated rich expertise in the treatment of this disease.He believes that the core pathogenesis of urinary discomfort in BPH is the dysfunction of Zang-Fu qi transformation leading to impaired water metabolism.The subsequent retention of phlegm-dampness and blood stasis is considered the key to gland enlargement and urinary obstruction.In treatment,he advocates restoring Zang-Fu qi transformation and has developed six basic therapeutic Methods:warming the kidney to transform qi,nourishing the kidney to clear qi,strengthening the spleen to benefit qi,soothing the liver to regulate qi,lifting the lid of the teapot(a metaphorical method to promote urination),and calming the heart to adjust qi.Additionally,he emphasizes the use of phlegm-removing and blood-activating drugs to reduce the size of the enlarged gland.The comprehensive treatment based on syndrome differentiation has achieved remarkable results.A typical case is attached to illustrate the argument.
