摘  要:目的探讨双孔法全胸腔镜心脏手术的手术质量并分析其学习曲线。方法回顾性分析连续完成的100例双孔法全胸腔镜心脏手术患者的临床资料,按照手术时间顺序分为A组、B组,每组患者50例。收集各组手术的体外循环时间、主动脉阻断时间、术后重症监护室(ICU)停留时间和术后呼吸机辅助时间等指标,比较手术效果。结果总手术失败率为5.0%。随着手术例数的增加,B组的体外循环时间指标优于A组,当手术例数达到50~60例时,体外循环时间稳定小于平均值。结论双孔法全胸腔镜心脏手术存在明显的学习曲线。术者进行了50~60例手术后,可以熟练掌握双孔法全胸腔镜心脏手术。Objective To monitor surgical quality and analyze learning curve of minimally invasive totally thoracoscopic cardiac surgery double orifice.Methods The clinical data of 100 consecutive patients with totally thoracoscopic cardiac surgery double orifice method completed were retrospectively analyzed,and were divided into group A and group B according to the chronological order of surgery,with 50 patients in each group.The extracorporeal circulation time,aortic block time,postoperative ICU stay time and postoperative ventilator-assisted time of each group were collected to compare the surgical effects.Results The total surgical failure rate was 5.0%.With the increase of surgical cases,the cardiopulmonary bypass time of the group B was significantly better than that of the group A.When surgical cases reached about 50-60 cases,the cardiopulmonary bypass time was stable and less than the average.Conclusion There is an obvious learning curve in thoracoscopic cardiac surgery double orifice.After accumulating 50 to 60 cases of operation,the surgeons were proficient in totally thoracoscopic cardiac surgery double orifice.
