摘 要:目的探讨智能心电门控技术辅助冠状动脉CT血管造影术(CTA)对肥胖患者冠心病及冠状动脉狭窄程度的诊断效果。方法选取我院2023年1月至2024年10月收集的80例肥胖冠心病患者,对所有患者分别进行常规冠状动脉CTA及智能心电门控技术辅助冠状动脉CTA诊断,分析2种诊断方式的CTA图像质量主观评分,并以冠状动脉造影作为“金标准”,分析不同诊断方式对肥胖冠心病患者冠状动脉特点、冠状动脉狭窄程度的诊断结果,分析智能心电门控技术辅助冠状动脉CTA对肥胖冠心病及冠状动脉狭窄程度的诊断价值。结果与常规冠状动脉CTA对比,智能心电门控技术辅助冠状动脉CTA冠状动脉节段、LCX、LAD、RCA相关图像质量主观评分更高(P<0.05);智能心电门控技术辅助冠状动脉CTA、冠状动脉CTA、冠状动脉造影诊断病变支数比较差异无统计学意义,病变类型分类差异无统计学意义,但冠状动脉CTA的病变检出综述低于冠状动脉造影(P<0.05),智能心电门控技术与冠状动脉造影相比差异无统计学意义(P>0.05);智能心电门控技术辅助冠状动脉CTA、冠状动脉CTA、冠状动脉造影均诊断出冠状动脉狭窄,但对于重度狭窄的诊断率具有一定差异,常规冠状动脉CTA对于重度狭窄的诊断率低于冠状动脉造影(P<0.05),智能心电门控技术与冠状动脉造影相比差异无统计学意义(P>0.05);智能心电门控技术辅助冠状动脉CTA对肥胖冠心病及冠状动脉狭窄程度的诊断准确度为95%、灵敏度为97%、阴性预测值为90%均高于常规冠状动脉CTA(P<0.05)。结论针对肥胖冠心病患者应用智能心电门控技术辅助冠状动脉CTA可提升CTA图像质量,并提升冠状动脉病变检出率与冠状动脉狭窄程度判断准确率,进一步提升冠心病及冠状动脉狭窄程度的诊断准确度与灵敏度。Objective To explore the diagnostic effect of intelligent electrocardiographic gating technology-assisted spiral CT coronary angiography(CTA)on coronary heart disease and the degree of coronary stenosis in obese patients.Methods Eighty obese patients with coronary heart disease collected from our hospital from January 2023 to October 2024 were selected for routine coronary artery CTA and intelligent electrocardiographic gating technology-assisted coronary artery CTA diagnosis.The subjective scores of CTA image quality for the two diagnostic methods were analyzed,and coronary angiography was used as the"gold standard"to analyze the diagnostic results of different diagnostic methods on the characteristics and degree of coronary artery stenosis in obese patients with coronary heart disease.The diagnostic value of intelligent electrocardiographic gating tech-nology assisted coronary artery CTA for obese coronary heart disease and the degree of coronary artery stenosis was analyzed.Results Compared with conventional coronary CTA,the subjective scores of image quality related to coronary artery segments,left circumflex coronary artery,left anterior descending,and right coronary artery were higher with the assistance of intelligent electrocardiographic gating technology in coronary CTA(P<0.05).There was no significant difference in the number of lesions diagnosed by intelligent electrocardiographic gating technology-assisted coronary CTA,coronary CTA,and coronary angiography,and no significant difference in the classification of lesion types.However,the summary of lesion detection by coronary CTA was lower than that by coronary angiography(P<0.05),and there was no significant difference between intelligent electrocardiographic gating technology and coronary angiography(P>0.05).Intelligent electrocardiographic gating technology assists in the diagnosis of coronary stenosis through coronary CTA,coronary CTA,and coronary angiography.However,there is a certain difference in the diagnostic rate of severe stenosis.Conventiona
