摘 要:目的分析珂立苏联合经鼻持续气道正压通气(nCPAP)辅助呼吸用于新生儿呼吸窘迫综合征(NRDS)的作用及对血气指标的影响。方法回顾性分析2022年1月—2024年6月114例NRDS患儿临床资料,依据治疗方式的不同进行分组,对照组予以nCPAP辅助呼吸治疗(n=54),观察组则实施nCPAP辅助呼吸联合珂立苏治疗(n=60),分析2组动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、吸入氧浓度(FiO_(2))水平、通气时间、氧疗时间、并发症总发生率以及呼吸机参数。结果医治前,两组血气指标水平对比差异无统计学意义(P>0.05),医治后观察组PaCO_(2)、FiO_(2)水平低于对照组,PaO_(2)水平高于对照组(P<0.05);观察组通气时间、氧疗时间明显短于对照组(P<0.05);观察组并发症总发生率为1.67%,明显低于对照组的16.67%(P<0.05);观察组气道峰压、呼气末正压、呼吸频率明显低于对照组(P<0.05)。结论珂立苏联合nCPAP治疗效果显著,既能改善NRDS患儿血气指标,缩短通气、氧疗时间,还有效降低并发症总发生率。Objective To analyze the role of Corisol combined with nasal continuous positive airway pressure(nCPAP)ventilation assisted respiration in neonatal respiratory distress syndrome(NRDS)and its effect on blood gas indexes.Methods The clinical data of 114 children with NRDS were retrospectively analyzed from January 2022 to June 2024,and they were grouped according to the different treatment modes:nCPAP-assisted respiratory treatment was given to a control group(n=54),and nCPAP-assisted respiratory treatment combined with Corisol was implemented in the observation group(n=60).Arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2)),inhaled oxygen concentration(FiO_(2))level,ventilation time,oxygen therapy time,total complication rate,and ventilator parameters were analyzed between the two groups.Results Before treatment,there was no difference in the comparison of the levels of blood gas indexes between the two groups(P>0.05).After treatment,the levels of PaCO_(2)and FiO_(2)in the observation group were lower than those in the control group,and the level of PaO_(2)was higher than that in the control group(P<0.05).The ventilation time and oxygen therapy time in the observation group were significantly shorter than those in the control group(P<0.05),and the total incidence of complications in the observation group was 1.67%,which was significantly lower than that in the control group(16.67%)(P<0.05).Peak airway pressure,positive end-expiratory pressure and respiratory rate of the observation group were significantly lower than those of the control group(P<0.05).Conclusion Corisol combined with nCPAP has a significant therapeutic effect,which can improve the blood gas indexes of children with NRDS,shorten the ventilation and oxygen therapy time,and also effectively reduce the total incidence of complications.
