摘  要:目的:研究肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、心肌肌钙蛋白I(cTnI)对一氧化碳中毒迟发性脑病(DEACMP)的预测价值。方法:选取2018年2月至2025年2月哈尔滨医科大学附属第二医院收治的119例急性一氧化碳中毒患者的临床资料进行回顾性研究。根据患者是否发生DEACMP,将患者分为预后良好组97例与预后不良组22例。比较年龄、性别及血常规、肝功能、D-二聚体、肌酐、CK、CK-MB、cTnI等实验室检查指标在两组间的差异,采用单因素和多因素Logistic回归方法分析患者预后的影响因素,并采用受试者工作特征(ROC)曲线分析影响因素对DEACMP发生的预测价值。结果:预后不良组CK、CK-MB和cTnI水平均显著高于预后良好组(P<0.001);分析结果显示,CK的ROC曲下面积(AUC)为0.886,截断值为375.000 U/L,灵敏度和特异度分别为0.909和0.753;CK-MB的AUC为0.947,截断值为11.500μg/L,灵敏度和特异度分别为1.000和0.825;cTnI的AUC为0.864,截断值为0.420μg/L,敏感度和特异度分别为0.864和0.733。结论:CK、CK-MB和c TnI这3种生物标志物对DEACMP有预测价值,有助于不良预后的早期识别和早期预防。Objective:To evaluate the predictive value of creatine kinase(CK),creatine kinase isoenzyme(CK-MB)and cardiac troponin I(cTnI)for delayed encephalopathy after acute carbon monoxide poisoning(DEACMP).Methods:A retrospective study was conducted on 119 patients with DEACMP admitted to the Second Affiliated Hospital of Harbin Medical University from February 2018 to February 2025.According to occurrence of DEACMP,the patients were divided into a good prognosis group(97 cases)and a poor prognosis group(22 cases).The differences of age,gender,blood routine,liver function,D-dimer,creatinine,CK,CK-MB,cTnI and other laboratory test indicators in the two groups were compared.Univariate and multivariate Logistic regression methods were used to analyze the influencing factors of patient prognosis,and receiver operating characteristics(ROC)curve was used to analyze the predictive value of influencing factors to the occurrence of DEACMP.Results:The levels of CK,CK-MB and cTnI in the poor prognosis group were significantly higher than those in the good prognosis group(P<0.001).The results showed that the area under the curve(AUC)of CK was 0.886,with a cutoff value of 375.000 U/L,and the sensitivity and specificity were 0.909 and 0.753,respectively.The AUC of CK-MB was 0.947,with a cutoff value of 11.500μg/L,and the sensitivity and specificity were 1.000 and 0.825,respectively.The AUC of cTnI was 0.864,with a cutoff value of 0.420μg/L,and the sensitivity and specificity were 0.864 and 0.733,respectively.Conclusions:Three biomarkers,CK,CK-MB and cTnI,have predictive value for DEACMP,which are helpful for early identification and early prevention of poor prognosis.
