基于SatScan的2017—2022年泸州市老年人原发性肝癌时空聚集性分析

摘　　要：目的了解2017—2022年泸州市老年人原发性肝癌患病时空分布特征及聚集性,为老年人原发性的规范化防治提供新思路。方法收集2017—2022年泸州市老年人原发性肝癌发病数据,利用Excel 2021软件进行率的计算,SaTScan v9.5软件进行时空聚集性分析,率的比较利用SPSS 24.0软件进行χ^(2)检验,检验水准α=0.05。结果2017—2022年泸州市老年人原发性肝癌发病率从85.48/10万上升至86.41/10万,男性发病率高于女性,发病率较高的县区主要分布在古蔺县、龙马潭区和泸县,从乡镇分布来看主要分布在泸州东南部的乡镇。时空扫描分析显示,存在五类聚集,包括4个县(区)的33个乡镇/街道,其中一类聚集中心为龙马潭区小市街道,聚集时间为2018—2020年;二类聚集中心为合江县榕山镇,聚集时间为2018—2020年;三类聚集中心为古蔺县龙山镇,聚集时间为2020年;四类聚集中心为古蔺县金兰街道,聚集时间为2017—2019年;五类聚集中心为泸县福集镇,聚集时间为2021—2022年。结论泸州市老年人原发性肝癌发病在部分乡镇(街道)仍存在时空聚集现象,尤其以龙马潭小市街道的聚集性最高。Objective To understand the characteristics of spatiotemporal aggregation of primary liver cancer among the elderly in Luzhou City from 2017 to 2022,and to provide evidence for standardized prevention and control of primary liver cancer among this population.Methods The data of primary liver cancer among the elderly in Luzhou City from 2017 to 2022 were collected.The rate was calculated by Excel 2021 and spatiotemporal aggregation analysis was performed with SatScan.The comparison of rates was conducted by Chi-square test with SPSS 24.0 software.And the test level wasα=0.05.Results From 2017 and 2022,the incidence rare of primary liver cancer among the elderly in Luzhou City increased from 85.48/105to 86.41/105.The incidence rate of males was higher than that of females.The counties/districts with high incidence were mainly distributed in Gulin County,Longmatan District and Lu County.From the perspective of townships,it is mainly distributed in the townships in the southeast of Luzhou City.The spatiotemporal scanning analysis showed that there were 5 types of cluster,including 33 towns/streets of 4 counties/districts.The first type of gathering center was Xiaoshi Street in Longmatan District,and the gathering time was from2018 to 2020.The second type of gathering center was Rongshan Town in Hejiang County,and the gathering time was from 2018 to 2020.The third type of gathering center was Longshan Town in Guling County,and the gathering time was 2020.The fourth type of gathering center was Jinlan Street in Guling County,and the gathering time was from 2017 to 2019.The five type of gathering center was Fuji Town in Lu County,and the gathering time was from 2021 to 2022.Conclusions The incidence of primary liver cancer among the elderly still existed the phenomenon of spatiotemporal aggregation in some towns/streets,especially in Xiaoshi street of Longmatan District.
