摘 要:目的:探讨基质金属蛋白酶-9(MMP-9)、转化生长因子-β1(TGF-β1)的表达水平与糖尿病合并白内障的相关性。方法:收集2022年1月~2024年3月在皖南医学院第二附属医院就诊并行白内障手术的患者138例(138只眼),男性64例(64只眼)、女性74例(74只眼),年龄(70.28±8.87)岁。根据是否患有糖尿病进行分组,糖尿病合并白内障患者为A组64例(64只眼),年龄相关性白内障患者为B组74例(74只眼),根据晶状体皮质混浊程度或核硬度进行组内分组,A1、B1组为晶状体中度混浊组,A2、B2组为晶状体重度混浊组;所有患者入院时抽取空腹肘静脉血,应用酶联免疫吸附法(ELISA)测定MMP-9、TGF-β1在两组患者血清中的表达水平,分析A组患者空腹血糖水平与MMP-9及TGF-β1水平的相关性。结果:A组入院时空腹血糖水平高于B组(P<0.001);两组患者血清中均存在MMP-9、TGF-β1的表达,A组血清中MMP-9、TGF-β1表达量高于B组(P<0.001);A1组血清中MMP-9、TGF-β1表达量低于A2组(P<0.001);B1组血清中MMP-9、TGF-β1表达量与B2组比较差异均无统计学意义(P>0.05)。相关性分析显示,A组患者空腹血糖水平与外周血清MMP-9(r=0.601,P<0.01)及TGF-β1(r=0.649,P<0.01)水平均呈正相关。结论:MMP-9、TGF-β1与糖尿病合并白内障患者的发生发展密切相关,且与血糖水平正相关。Objective:To investigate the correlation between the expression levels of matrix metalloproteinase-9(MMP-9)and transforming growth factor-β1(TGF-β1)and the occurrence of diabetes complicated with cataract.Methods:138 patients(138 eyes)undergone cataract surgery in our hospital were included from January 2022 to March 2024.Of the patients,64 were males(64 eyes),and 74 females(74 eyes).The average age was(70.28±8.87)years.By the presence of concurrent conditions,the patients were then divided into group A(n=64;64 eyes),in which the patients were diabetic concomitant with cataract,and group B(n=74;74 eyes),whose cataract was associated with the older age.Based on the degree of cortical opacity or nuclear sclerosis of the lens,the cases were then sub-grouped into group A1 and B1(moderate lens opacity),and A2 and B2(severe lens opacity).Fasting blood was taken from all patients via the elbow vein upon admission,and measured using enzyme-linked immunosorbent assay(ELISA)for the expression levels of serum MMP-9 and TGF-β1 in both groups,and the correlation between the levels of fasting blood glucose and MMP-9 and TGF-β1 was analyzed in patients in group A.Results:The fasting blood glucose level of group A at admission was higher than that of group B(P<0.001).MMP-9 and TGF-β1 were expressed in the sera of both groups of patients,yet the level was higher in group A than in group B(P<0.001).The expression levels of MMP-9 and TGF-β1 in the serum of group A1 were lower than those in group A2(P<0.001),whereas the difference was insignificant in the serum MMP-9 and TGF-β1 levels between group B1 and B2(P>0.05).Correlation analysis revealed that the fasting blood glucose levels of patients in group A showed significant positive correlation with both the serum levels of MMP-9(r=0.601,P<0.01)and TGF-β1(r=0.649,P<0.01).Conclusion:The expression levels of MMP-9 and TGF-β1 are closely related to the occurrence and developments of diabetes complicated with cataract,and are positively correlated with patients′blood glucose
