慢性病共病患者服药依从性现状及影响因素分析

摘　　要：目的:分析慢性病共病患者服药依从性影响因素,为其合理用药提供依据。方法:选取2023年5月~2023年10月芜湖市某三甲医院644例慢性病共病患者进行问卷调查,包括社会人口学资料调查表、服药依从性量表、衰弱量表及健康素养量表。结果:644例患者中,76.1%患者用药依从性较差。多因素Logistic回归分析显示,女性(OR=1.968,95%CI:1.304~2.970)、健康素养水平低(OR=1.904,95%CI:1.175~3.085)、衰弱(OR=1.891,95%CI:1.226~2.916)均是慢性共病患者服药依从性的危险因素(P<0.05),亲属是医护人员(OR=0.627,95%CI:0.417~0.941)、完全自理(OR=0.107,95%CI:0.014~0.807)是慢性共病患者服药依从性的保护因素(P<0.05)。结论:慢性共病患者用药依从性较低。医护人员应针对患者衰弱、健康素养等问题,采取针对性护理干预措施预防,提高慢性共病患者服药依从性。Objective:To analyze the influencing factors of medication adherence in patients with chronic comorbidities for evidences to use the drug in rational fashion.Methods:In total,644 patients with chronic comorbidities were recruited from a tertiary hospital in Wuhu City between May and October of 2023 and underwent questionnaire survey.The questionnaire consisted of sociodemographic information,Medication Adherence Scale,Frailty Scale and Health Literacy Scale.Results:Of the 644 patients,76.1%had poor medication adherence.Logistic regression analysis showed that women(OR=1.968,95%CI:1.304-2.970),lower health literacy level(OR=1.904,95%CI:1.175-3.085)and frailty(OR=1.891,95%CI:1.226-2.916)were risk factors for medication compliance in patients with chronic comorbidities(P<0.05).The patients′relatives were health providers(OR=0.627,95%CI:0.417-0.941)and those with full self-care ability(OR=0.107,95%CI:0.014-0.807)were protective factors for the medication adherence(P<0.05).Conclusion:Patients with chronic comorbidities have poor medication compliance.Medical staff should take targeted nursing interventions for the patientswith chronic comorbidities,particularly those of frailty and with lower health literacy,so as to improve their medication compliance.
