摘 要:目的:探究相对腹部按摩与三维呼吸锻炼,三维呼吸联合生物压力反馈仪预防经椎间孔入路腰椎体融合术(TLIF)后患者便秘的效果。方法:将90例患者随机分为3组,每组30例,除常规护理干预外,A组给予腹部按摩法,B组给予三维呼吸训练,C组给予三维呼吸联合生物压力反馈仪训练,比较3组患者术后便秘发生率、首次肛门排气、排便时间、术前及术后3 d和5 d患者的便秘总积分差异,患者锻炼的依从性及护理成本。结果:C组患者便秘发生率(48.83%)低于A组(75.86%),P<0.05;C组首次排气时间早于A组,C组排便时间早于A、B组(P<0.05);相对B、A组,C组患者的依从性较高(P<0.05);C组相对B组的护理成本更低(P<0.05);3组便秘症状评分比较,与A组比较,C组术后3 d和5 d治疗的效果更好(P<0.05);与B组比较,C组术后5 d治疗的效果更好(P<0.05)。与术前比较,3组术后3 d和A、B组术后5 d症状更严重(P<0.05);与术后3 d比较,3组术后5 d症状更好(P<0.05)。结论:三维呼吸联合生物压力反馈仪锻炼可改善TLIF术后患者便秘症状、降低便秘发生率、提高患者锻炼依从性及缩减护理成本的效果更加显著,可在临床推广使用。Objective:To observe the clinical effect of abdominal massage plus three-dimensional respiratory exercise and three-dimensional respiratory exercise combined with biofeedback instrument on preventing constipation in patients after lumbar interbody fusion via intervertebral foramen.Methods:Ninety patients were evenly randomized to group A(treated by abdominal massage),group B(treated by three-dimensional respiratory exercise)and group C(managed by three-dimensional respiratory exercise combined with biofeedback instrument).Then the three groups were compared regarding the incidence of constipation,time of first anal exhaust and defecation,differences in total constipation scores before and after surgery in the 3 rd and 5 th day,patient compliance,and nursing costs.Results:The incidence of constipation was lower in group C than in group A(48.83%vs.75.86%,P<0.05).The first exhaust time in group C was significantly earlier than that in group A,and the defecation time in group C was earlier than that in groups A and B(P<0.05).The compliance of patients in group C was higher than that in group B and group A(P<0.05),and the nursing cost was lower in group C than in group B(P<0.05).Comparison of constipation symptom scores among the three groups indicated that group C had better treatment effects at 3 and 5 days after surgery compared to group A(P<005),and the scoring was better in group C than in group B by the effect at day 5 after operation(P<0.05).Compared with preoperative data,the symptoms were worse in group A and B at day 3 and 5 days after surgery(P<0.05),yet the symptoms were better in the 5 th day than those in the 3 rd day following surgery among the three group(P<0.05).Conclusion:Three-dimensional respiratory exercise combined with biofeedback instrument may be recommended in clinical application because it can significantly improve constipation symptoms,reduce the incidence of constipation,improve patient compliance,and reduce nursing costs for patients after lumbar interbody fusion via intervertebral foram
