半侧头高卧位对老年骨折患者术后夜间呼吸功能的影响

摘　　要：目的:探讨半侧头高卧位对老年骨折患者术后夜间呼吸功能的影响。方法:纳入全椒县人民医院60例老年骨折手术患者,1∶1随机分为观察组和对照组。观察组夜间45°半侧且头高15°卧位,对照组头高30°仰卧位。记录两组术后3 d夜间血氧饱和度(SpO_(2))、第1夜间膈肌移动度、第1天和第2天排痰量、术后肺部并发症(PPCs)发生情况及住院天数等。结果:观察组术后3 d夜间SpO_(2)曲线下面积、第1夜膈肌移动度、第1天排痰量均高于对照组(P<0.05)。观察组住院时间更短(P<0.05)。结论:半侧头高卧位能改善老年骨折患者夜间呼吸功能,促进术后24 h内排痰,减少PPCs。Objective:To observe the impact of high hemi-lateral head recumbent position the postoperative respiration at nighttime in elderly patients with fractures.Methods:Sixty elderly patients with fractures following surgery were included,and randomly divided into the observational group and the control group in a 1:1 ratio.The patients in the observational group were maintained at a 45°semi-recumbent position with the head heightened by 15°,and those in the control group was placed in a supine position with the head elevated by 30°during night after surgery.The postoperative data,including nighttime saturation of peripheral oxygen(SpO_(2))for three consecutive nights,diaphragmatic mobility on the first night,sputum volume on the first and second days,the incidence of pulmonary complications and the duration of hospital stay,were documented in the two groups of patients.Results:The observational group had higher values for the area under the SpO_(2) curve during the three postoperative nights,diaphragmatic mobility on the first night,and sputum volume on the first day,yet shorter hospital stay after surgery compared to the control group(all P<0.05).Conclusion:High hemi-lateral head recumbent position can improve the respiratory function at nighttime in elderly fracture patients,promote expectoration within 24 hours after surgery,and reduce postoperative pulmonary complications.
