摘  要:目的了解益阳地区泛耐药鲍曼不动杆菌(XDRAB)碳青霉烯酶耐药基因流行特点。方法采用VITEK-2 Compact细菌鉴定药敏仪进行细菌鉴定及药敏试验,采用PCR方法检测7种碳青霉烯酶基因(bla_(OXA-23)、bla_(OXA-51)、bla_(OXA-58)、bla_(IMP)、bla_(KPC)、bla_(VIM)、bla_(NDM))。结果105株XDRAB对米诺环素、多西环素的敏感率分别为72.4%、30.5%,对黏菌素、头孢哌酮/舒巴坦的中介率分别为100.0%、33.3%。105株XDRAB碳青霉烯酶基因bla_(OXA-23)、bla_(OXA-51)检出率均为100.0%,bla_(NDM)检出率为24.8%,未检出bla_(OXA-58)、bla_(IMP)、bla_(KPC)、bla_(VIM)。结论目前治疗XDRAB的药物有限,bla_(OXA-23)、bla_(OXA-51)、bla_(NDM)为本地区碳青霉烯酶主要耐药基因。Objective To understand the epidemiological characteristics of carbapenemase resistance genes of extensively drug-resistant Acinetobacter baumannii(XDRAB)in Yiyang area.Methods VITEK-2 Compact bacterial identification and drug susceptibility testing were performed,and PCR method was used to detect 7 carbapenemase genes(bla_(OXA-23),bla_(OXA-51),bla_(OXA-58),bla_(IMP),bla_(KPC),bla_(VIM),bla_(NDM)).Results The sensitivity rates of 105 strains of XDRAB to minocycline and doxycycline were 72.4%and 30.5%,respectively.The mediation rates to colistin and cefoperazone/sulbactam were 100.0%and 33.3%,respectively.The detection rates of the carbapenemase genes bla_(OXA-23) and bla_(OXA-51) in 105 strains of XDRAB were 100.0%,and the detection rate of bla_(NDM) was 24.8%.No bla_(OXA-58),bla_(IMP),bla_(KPC),or bla_(VIM) were detected.Conclusion Currently,there are limited drugs available in the treatment of XDRAB,and bla_(OXA-23),bla_(OXA-51),and bla_(NDM) are the main carbapenemase resistant genes in this area.
