集中带量采购政策对某三甲医院口服降糖药物使用的影响

摘　　要：目的对比分析实施国家集中带量采购(简称集采)政策前后新疆医科大学第五附属医院(本院)口服降糖药物的使用变化。方法收集本院2019年1月至2023年12月口服降糖药物的品种、数量、使用金额,计算片数构成比、用药频度、日均治疗费用等,分析数据变化。结果实施国家集采政策后,本院集采口服降糖药物品种数量占比自2021年起,均超过50%;集采药品价格均明显下降,二甲双胍片、格列美脲、瑞格列奈片和阿卡波糖片单片价格降幅分别达到96.88%、95.38%、89.40%、83.49%;口服降糖药物金额占比逐年下降,2023年占比降至1.58%;口服降糖药物日均治疗费用下降,降幅最明显的是格列美脲和二甲双胍片,分别为97.26%、95.05%;集采口服降糖药物片数构成比逐年上升,2023年达到88.15%。结论国家集采政策的实施,有效降低了口服降糖药物的价格,切实减轻了糖尿病患者用药的经济负担,在临床被广泛使用。Objective To compare and analyze changes in the use of oral hypoglycemic drugs at the Fifth Affiliated Hospital of Xinjiang Medical University(our hospital)before and after the implementation of the national centralized volume-based procurement policy(centralized procurement).Methods Data on the varieties,quantities,and usage amounts of oral hypoglycemic drugs in our hospital from January 2019 to December 2023 were collected.The constituent ratio of number of tablets,medication frequency,average daily treatment cost and other parameters were calculated,and changes in the data were analyzed.Results Following the implementation of the national centralized volume-based procurement policy,it was observed that the proportion of the number of varieties of oral hypoglycemic drugs centrally procured by our hospital had exceeded 50%since 2021.Significant decreases in the prices of the centrally procured drugs were noted,with the price of metformin tablets,glimepiride,repaglinide tablets,and acarbose tablets decreasing by up to 96.88%,95.38%,89.40%,and 83.49%per tablet,respectively.The proportion of expenses on oral hypoglycemic drugs decreased year by year,reaching 1.58%in 2023.The average daily treatment cost of oral hypoglycemic drugs also decreased,with the most significant reductions observed in glimepiride and metformin tablets,which saw decreases of 97.26%and 95.05%,respectively.Additionally,the constituent ratio of number of tablets of oral hypoglycemic drugs obtained by centralized procurement increased annually,reaching 88.15%in 2023.Conclusion It can be concluded that the implementation of the national centralized procurement policy has effectively reduced the prices of oral hypoglycemic drugs,alleviating the economic burden of medication for diabetic patients and leading to widespread clinical use.
