摘 要:目的探讨钠-葡萄糖协同转运蛋白2(SGLT-2)抑制剂对2型糖尿病(T2DM)患者血清鸢尾素水平的影响。方法选取2022年1—12月于潍坊市人民医院内分泌科治疗的T2DM患者50例,其中SGLT-2抑制剂治疗组(二甲双胍+SGLT-2抑制剂)25例、非SGLT-2抑制剂治疗组25例(二甲双胍+其他口服降糖药物)。按照体重指数(BMI)分组后,在不同BMI人群中,比较应用SGLT-2抑制剂对受试者血清鸢尾素水平的影响。应用Spearman相关分析明确影响血清鸢尾素水平的相关因素。多元逐步回归分析探讨血清鸢尾素水平的独立危险因素。结果在BMI<24 kg/m^(2)的T2DM患者中,与非SGLT-2抑制剂治疗组比较,SGLT-2抑制剂治疗组患者的血清鸢尾素水平明显降低(P<0.05)。相关分析结果显示,血清鸢尾素水平与应用SGLT-2抑制剂治疗呈负相关。多元逐步回归分析结果显示,在BMI<24 kg/m^(2)的T2DM患者中,SGLT-2抑制剂和年龄是血清鸢尾素水平的独立负性影响因素。结论在BMI<24 kg/m^(2)的T2DM患者中,SGLT-2抑制剂是血清鸢尾素水平的独立影响因素,其对肌肉代谢可能存在负性影响。Objective To investigate the impacts of sodium-dependent glucose transporters 2(SGLT-2)inhibitor on serum irisin levels in patients with type 2 diabetes mellitus(T2DM).Methods A total of 50 T2DM patients admitted to and treated in the Department of Endocrinology of Weifang People’s Hospital from January to December 2022 were selected,and they were divided into the SGLT-2 inhibitor treatment group(n=25,metformin+SGLT-2 inhibitor)and the non SGLT-2 inhibitor treatment group(n=25,metformin+other oral hypoglycemic drugs).After grouping according to body mass index(BMI),the impacts of SGLT-2 inhibitors on serum irisin levels were compared in different BMI populations.Spearman correlation analysis was used to identify the relevant factors impacting serum irisin levels.Multiple stepwise regression analysis was used to investigate independent risk factors for serum irisin levels.Results In T2DM patients with BMI<24 kg/m^(2),compared with the non SGLT-2 inhibitor treatment group,the serum irisin level of patients in the SGLT-2 inhibitor treatment group was significantly reduced(P<0.05).The correlation analysis results showed that serum irisin levels were negatively correlated with the use of SGLT-2 inhibitors for treatment.The results of multiple stepwise regression analysis showed that in T2DM patients with BMI<24 kg/m^(2),SGLT-2 inhibitors and age were independent negative factors impacting serum irisin levels.Conclusion SGLT-2 inhibitors are an independent factor impacting serum irisin levels in T2DM patients with BMI<24 kg/m^(2),and may have a negative impact on muscle metabolism.
