温胆通脉汤联合氟哌噻吨美利曲辛片治疗痰瘀型脑卒中后抑郁临床研究

摘　　要：目的探讨温胆通脉汤联合氟哌噻吨美利曲辛片对痰瘀型脑卒中后抑郁的影响。方法选取枣庄市中医医院2024年1—7月脑病康复科收治的痰瘀型脑卒中后抑郁患者60例,根据随机数表法将其分为观察组与对照组,每组各30例。两组均进行基础治疗,对照组予氟哌噻吨美利曲辛片治疗,观察组在对照组基础上加服温胆通脉汤治疗。比较两组患者17项汉密尔顿抑郁量表(HAMD-17)、匹兹堡睡眠质量指数量表(PSQI)、日常生活能力量表(BI)、美国国立卫生研究院脑卒中量表(NHISS)评分及临床疗效。结果治疗8周后,观察组HAMD-17、PSQI、NHISS评分均低于对照组,BI评分高于对照组,且临床疗效大于对照组,差异有统计学意义(P<0.05)。结论温胆通脉汤联合氟哌噻吨美利曲辛片是痰瘀型脑卒中后抑郁的有效治疗方案,可改善患者抑郁症状及睡眠质量,提高日常生活能力,降低神经功能缺损程度。Objective To investigate the impact of Wendan Tongmai Decoction combined with flupentixiton and meritocine tablets in the treatment of post-stroke depression of phlegm and blood stasis type.Methods A total of 60 patients with post-stroke depression of phlegm and blood stasis type admitted to the Department of Encephalopathy Rehabilitation of Zaozhuang Hospital of Traditional Chinese Medicine from January to July 2024 were selected.They were divided into the observation group(n=30)and the control group(n=30)according to the random number table method.Both groups were given basic treatment.The control group was treated with flupentixol and melitracen tablets,while the observation group was treated with Wendan Tongmai Decoction in addition to the control group.T he 17-item Hamilton depression rating scale(HAMD-17),Pittsburgh sleep quality index(PSQI),Barthel index(BI),National Institutes of health stroke scale(NHISS)scores,and clinical efficacy between two groups of patients were compared.Results After 8 weeks of treatment,the HAMD-17,PSQI,and NHISS scores of the observation group were all lower than those of the control group,while the BI score was higher than that of the control group.The clinical efficacy was remarkably higher than that of the control group,with statistically significant differences(P<0.05).Conclusion The combination of Wendan Tongmai Decoction and flupentixol and melitracen tablets is an effective treatment for post-stroke depression of phlegm and blood stasis type.It can improve patients’depressive symptoms and sleep quality,enhance their daily living abilities,and reduce the degree of neurological dysfunction.
