肿瘤化疗患者应用PICC期间给予全程护理对导管相关感染并发症的影响研究

摘　　要：目的研究与探讨肿瘤化疗患者应用经外周静脉穿刺中心静脉置管术(PICC)期间给予全程护理对导管相关感染并发症危险因素的影响。方法选取2021年1月至2023年6月福建省莆田市第一医院肿瘤内科收治的192例肿瘤化疗患者进行研究,依据随机数表法分为观察和对照两组,每组各96例。两组均应用PICC,对照组给予常规护理,观察组在对照组的基础上给予全程护理,比较两组导管相关感染并发症危险因素的影响。结果观察组的导管相关感染并发症总发生率为5.21%,明显低于对照组的16.67%,差异有统计学意义(P<0.05)。单因素分析结果显示,置管时间、维护间隔时间、间歇期体温、激素应用情况差异有统计学意义(P<0.05)。多因素logistic回归分析结果显示,患者置管时间≥3个月、间歇期体温≥38℃、维护间隔时间≥7 d、使用激素为导管相关感染并发症的独立危险因素(P<0.05)。观察组的护理总满意度为98.96%,明显高于对照组的84.37%,差异有统计学意义(P<0.05)。结论肿瘤化疗患者应用PICC期间给予全程护理对导管相关感染并发症有所影响,能够降低并发症发生率,提高患者满意度,适合推广与应用。Objective To study and investigate the impact of holistic nursing on the risks of catheterrelated infection complications during peripherally inserted central catheter(PICC)application in tumor chemotherapy patients.Methods A total of 192 cases of tumor chemotherapy patients admitted to and treated in the Department of Tumor Medicine of the First Hospital of Putian from January 2021 to June 2023 were studied,and they were divided into the observation group(n=96)and the control group(n=96)according to the random number table method.PICC was used in both groups.The control group was treated with conventional nursing,while the observation group was treated with holistic nursing on the basis of the control group.The impact of risk factors of catheter-related infection complications between the two groups was compared.Results The total incidence of catheter-related infection complications in the observation group was 5.21%,obviously lower than 16.67%in the control group,with statistically significant difference(P<0.05).The results of univariate analysis showed that there were statistically significant differences in fours items of the intubation time,maintenance interval,intermittent body temperature and hormone application(P<0.05).The results of multivariate logistic regression analysis showed that patients’indwelling time was≥3 months,intermittent body temperature was≥38℃,maintenance interval was≥7 d,and hormone use were the independent risk factors for catheter-related infection complications(P<0.05).The total nursing satisfaction of the observation group reached 98.96%,obviously higher than 84.37%of the control group,with statistically significant difference(P<0.05).Conclusion During PICC application,the impact of holistic nursing on the risk factors of catheter-related infection complications in tumor chemotherapy patients can reduce the incidence of complications and improve patient satisfaction,which is suitable for promotion and application.
