褪黑素与针灸治疗早发性卵巢功能不全的疗效及对血清性激素、卵巢动力学的影响

摘　　要：目的分析褪黑素结合针灸治疗早发性卵巢功能不全(POI)的临床疗效。方法选取2022年6月-2023年6月我院收治的POI患者168例,经随机数字表法分为联合组与对照组,各84例。两组基础治疗均为激素替代疗法,对照组在此基础上使用针灸治疗,而联合组在对照组基础上使用褪黑素片治疗,比较两组临床疗效、血清性激素[雌二醇(E_(2))、黄体生成素(LH)、卵泡刺激素(FSH)、抗缪勒管激素(AMH)]、窦卵泡计数(AFC)、妊娠相关蛋白及卵巢血流动力学[卵巢动脉舒张末期流速(EDV)、收缩期峰值流速(PSV)、搏动指数(PI)、阻力指数(RI)]。结果两组治疗后E_(2)、AMH、AFC数目、妊娠相关蛋白水平、EDV、PSV高于治疗前,且联合组高于对照组(P<0.05);两组治疗后LH、FSH、PI、RI均低于治疗前,且联合组均低于对照组(P<0.05)。结论褪黑素结合针灸治疗POI的疗效较好,可改善患者血清性激素、窦卵泡计数、妊娠相关蛋白及卵巢血流动力学,进一步提升卵巢功能。Objective To analyze the curative effect of melatonin combined with acupuncture and moxibustion in the treatment of premature ovarian insufficiency(POI).Methods A total of 168 patients with POI admitted to our hospital from June 2022 to June 2023 were divided into combined group and control group by random number table method,with 84 patients in each group.The basic treatment of the two groups was hormone replacement therapy.The control group was treated with acupuncture and moxibustion on this basis,while the combined group was treated with melatonin tablets on the basis of the control group.The clinical efficacy,serum sex hormones[estradiol(E_(2)),luteinizing hormone(LH),follicle stimulating hormone(FSH),anti-Müllerian hormone(AMH)],antral follicle count(AFC),pregnancy-related protein and ovarian hemodynamics[ovarian artery end-diastolic velocity(EDV),peak systolic velocity(PSV),pulsatility index(PI),resistance index(RI)]were compared between the two groups.Results After treatment,the levels of E_(2),AMH,AFC,pregnancy-related protein level,EDV,and PSV in both groups were higher than before treatment,and those in the combined group were higher than those in the control group(P<0.05).After treatment,LH,FSH,PI,and RI in both groups were higher than before treatment,and those in the combined group were lower than those in the control group(P<0.05).Conclusion Melatonin combined with acupuncture and moxibustion is effective in the treatment of POI,which can improve serum sex hormone,antral follicle count,pregnancy-related protein and ovarian hemodynamics,and further improve ovarian function.
