药师针对性干预的处方审核模式在静脉用药集中调配中心合理用药中的应用效果

摘　　要：目的:分析药师针对性干预的处方审核模式在静脉用药集中调配中心(PIVAS)合理用药性中的应用效果。方法:选取徐州医科大学附属宿迁医院药学部2022年1—12月PIVAS实施常规处方审核模式的处方500张作为对照组,选取2023年1—12月PIVAS实施药师针对性干预的处方审核模式的处方500张作为实施组。对比两组不合理处方、管理质量、患者不良反应发生情况。结果:实施组药物剂量不合理、溶媒选择不合理、给药顺序不合理、给药途径不合理、给药间隔时间不合理占比低于对照组(P<0.001)。实施组药学服务、处方调配、处方配置流程管理、联合用药管理评分均比对照组高(P<0.001)。相比于对照组,实施组患者不良反应发生率更低(P<0.001)。结论:药师针对性干预在PIVAS合理用药中的应用效果较好,能够提高管理质量,减少不合理处方,降低患者用药不良反应发生率。Objective:To analyze the application effect of prescription review mode with pharmacists'targeted intervention in rational medication of pharmacy intravenous admixture services(PIVAS).Methods:A total of 500 prescriptions under routine prescription review mode of PIVAS in Pharmacy Department at Suqian Hospital Affiliated to Xuzhou Medical University from January 2022 to December 2022 were selected as the control group.A total of 500 prescriptions under prescription review mode with pharmacists'targeted intervention of PIVAS from January 2023 to December 2023 were selected as the implementation group.The irrational prescription,management quality and occurrence of adverse reactions were compared between two groups.Results:The proportions of unreasonable drug dose,unreasonable vehicle selection,unreasonable administration sequence,unreasonable route of administration,and unreasonable administration interval in the implementation group were lower than those in the control group(P<0.001).The scores of pharmacy service,prescription dispensing,prescription allocation process management and combined medication management in the implementation group were higher than those in the control group(P<0.001).The incidence of adverse reactions in the implementation group was lower than that in the control group(P<0.001).Conclusion:The application effect of pharmacist's targeted intervention in the rational medication of PIVAS is good,which can improve the quality of management,reduce irrational prescription,and reduce the incidence of adverse drug reactions in patients.
