甘精胰岛素联合阿卡波糖治疗老年2型糖尿病的临床效果

摘　　要：目的:探讨甘精胰岛素联合阿卡波糖治疗老年2型糖尿病的临床效果。方法:选择2021年7月—2023年8月新疆生产建设兵团第一师医院收治的老年2型糖尿病患者370例进行研究,应用随机数字表法分为观察组(n=185)、对照组(n=185)。对照组采取阿卡波糖片治疗,观察组采取甘精胰岛素联合阿卡波糖片治疗。比较两组患者治疗效果、血糖水平、抵抗素水平、胰岛素抵抗指数(HOMA-IR)。结果:观察组治疗总有效率比对照组高(P<0.05)。治疗前,两组空腹血糖(FBG)、餐后2 h血糖(2 hPBG)、糖化血红蛋白(HbA1c)水平比较,差异无统计学意义(P>0.05);治疗后,两组FBG、2 hPBG、HbA1c水平降低,且观察组低于对照组(P<0.05)。治疗前,两组抵抗素水平、HOMA-IR比较,差异无统计学意义(P>0.05);治疗后,两组抵抗素水平、HOMA-IR降低,且观察组低于对照组(P<0.05)。结论:甘精胰岛素联合阿卡波糖治疗老年2型糖尿病的临床效果显著,能够降低患者血糖水平,改善胰岛功能。Objective:To investigate the clinical effect of insulin glargine combined with acarbose in the treatment of senile type 2 diabetes mellitus.Methods:A total of 370 elderly patients with type 2 diabetes mellitus admitted to the Xinjiang Production and Construction Corps First Division Hospital from July 2021 to August 2023 were selected for the study.They were divided into observation group(n=185)and control group(n=185)by random number table method.Control group was treated with acarbose tablets and observation group was treated with insulin glargine combined with acarbose tablets.The therapeutic effect,blood glucose level,resistin level,and insulin resistance index(HOMA-IR)were compared between the two groups.Results:The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).Before treatment,there was no statistically significant difference in fasting blood glucose(FBG),2 h postprandial blood glucose(2 hPBG)and glycosylated hemoglobin(HbA1c)levels between the two groups(P>0.05);After treatment,the levels of FBG,2 hPBG and HbA1c in the two groups decreased,and the levels in the observation group were lower than those in the control group(P<0.05).Before treatment,there was no statistically significant difference in the levels of resistin and HOMA-IR between the two groups(P>0.05).After treatment,the levels of resistin and HOMA-IR in the two groups decreased,and the levels in the observation group were lower than those in the control group(P<0.05).Conclusion:Insulin glargine combined with acarbose has significant clinical effect in the treatment of senile type 2 diabetes mellitus,and can reduce blood glucose levels and improve pancreatic islet function.
