卡格列净联合常规疗法治疗2型糖尿病合并慢性心力衰竭的效果分析

摘　　要：目的:分析卡格列净联合常规疗法治疗2型糖尿病合并慢性心力衰竭的效果。方法:选取2021年6月—2024年7月北京市回民医院收治的120例2型糖尿病合并慢性心力衰竭患者进行研究,按照随机数字表法分为对照组和试验组,每组60例。对照组接受常规治疗,试验组在对照组基础上接受卡格列净治疗。对比两组血糖控制情况、心功能指标、炎性因子水平。结果:治疗前,两组B型钠尿肽(BNP)水平、左室射血分数(LVEF)、左室舒张末压(LVEDP)、6min步行距离比较,差异无统计学意义(P>0.05);治疗后,两组BNP、LVEDP降低,LVEF升高,6min步行距离延长,且试验组改善幅度大于对照组(P<0.05)。治疗前,两组高敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平比较,差异无统计学意义(P>0.05);治疗后,两组hs-CRP、IL-6、TNF-α水平降低,且试验组低于对照组(P<0.05)。治疗前,两组空腹血糖、餐后2 h血糖水平比较,差异无统计学意义(P>0.05);治疗后,两组空腹血糖、餐后2 h血糖水平降低,且试验组低于对照组(P<0.05)。结论:卡格列净联合常规疗法治疗2型糖尿病合并慢性心力衰竭的效果确切,有助于改善患者心功能,控制血糖水平,减轻炎症。Objective:To analyze the effect of canagliflozin combined with conventional therapy in treatment of type 2 diabetes mellitus complicated with chronic heart failure.Methods:A total of 120 patients with type 2 diabetes mellitus combined with chronic heart failure admitted to the Beijing Hui Min Hospital from June 2021 to July 2024 were selected for the study.They were divided into control group and experimental group according to random number table method,with 60 cases in each group.The control group received conventional treatment,and the experimental group received canagliflozin treatment on the basis of the control group.Blood glucose control,cardiac function index and levels of inflammatory factors were compared between the two groups.Results:Before treatment,there was no statistically significant difference in the level of B-type natriuretic peptide(BNP),left ventricular ejection fraction(LVEF),left ventricular end-diastolic pressure(LVEDP)and 6-min walking distance between the two groups(P>0.05).After treatment,BNP and LVEDP decreased,LVEF increased,and 6-min walking distance prolonged in two groups,and the improvement in the experimental group was greater than that in the control group(P<0.05).Before treatment,there was no statistically significant difference in the levels of high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)between the two groups(P>0.05).After treatment,the levels of hs-CRP,IL-6 and TNF-αdecreased in the two groups,and the levels in the experimental group were lower than those in the control group(P<0.05).Before treatment,there was no statistically significant difference in the levels of fasting blood glucose and 2 h postprandial blood glucose between the two groups(P>0.05).After treatment,the levels of fasting blood glucose and 2 h postprandial blood glucose levels decreased in the two groups,and the levels in the experimental group were lower than those in the control group(P<0.05).Conclusion:Canagliflozin combined with conventional thera
