宫腔镜下电切术与宫腔镜下刮宫术治疗异常子宫出血的效果比较

摘　　要：目的:对比宫腔镜下电切术与宫腔镜下刮宫术治疗异常子宫出血的效果。方法:选取2023年7月—2024年8月菏泽市郓城县妇幼保健院收治的异常子宫出血患者70例作为研究对象,随机分为对照组与观察组,各35例。对照组实施宫腔镜下刮宫术,观察组实施宫腔镜下电切术。比较两组治疗效果。结果:观察组手术时间、住院时间短于对照组,术中出血量少于对照组(P<0.001)。术后3个月,两组月经量减少,且观察组少于对照组(P<0.05);两组经期时间缩短,且观察组短于对照组(P<0.05);两组子宫内膜厚度减小,且观察组小于对照组(P<0.05)。术后3个月,两组血细胞比容、血红蛋白水平升高,且观察组高于对照组(P<0.05)。观察组复发率低于对照组(P=0.040)。结论:相较于宫腔镜下刮宫术,宫腔镜下电切术治疗异常子宫出血的效果更佳,其创伤小、手术时间短,能够改善患者术后月经情况、血液指标,降低复发率。Objective:To compare the effect of hysteroscopic electroresection and hysteroscopic curettage in treatment of abnormal uterine bleeding.Methods:A total of 70 patients with abnormal uterine bleeding admitted to Heze Yuncheng County Maternal and Child Health Care Hospital from July 2023 to August 2024 were selected as the study subjects.They were randomly divided into control group and observation group,with 35 cases in each group.The control group underwent hysteroscopic curettage and the observation group underwent hysteroscopic electroresection.The treatment effects were compared between two groups.Results:The operation time and the length of hospital stay in the observation group were shorter than those in the control group,and the intraoperative blood loss in the observation group was less than that in the control group(P<0.001).At 3 months after operation,the menstrual blood volume decreased in the two groups,and this parameter in the observation group was less than that in the control group(P<0.05).The menstrual period duration shortened in the two groups,and this parameter in the observation group was shorter than that in the control group(P<0.05).The endometrial thickness decreased in the two groups,and this parameter in the observation group was lower than that in the control group(P<0.05).At 3 months after operation,hematocrit and hemoglobin levels increased in the two groups,and the levels in the observation group were higher than those in the control group(P<0.05).The recurrence rate in the observation group was lower than that in the control group(P=0.040).Conclusion:Compared with hysteroscopic curettage,hysteroscopic electroresection has a better effect in the treatment of abnormal uterine bleeding,induces fewer trauma and has a shorter operation time,and can improve the postoperative menstrual condition and blood indexes of patients,and reduce the recurrence rate.
