摘 要:目的:探讨25-羟基维生素D[25-(OH)D]、甲状旁腺激素(PTH)水平与老年人骨密度的相关性。方法:选取2022年1—10月于北京市大兴区旧宫医院行骨质疏松筛查的老年人546例作为研究对象,均接受25(OH)D、PTH检测及骨密度检查,并根据骨密度检测结果分为骨密度正常组(n=211,T>-1)、低骨密度组(n=252,-2.5≤T≤-1)、骨质疏松组(n=83,T<-2.5)。比较三组25-(OH)D、PTH水平,分析25-(OH)D、PTH水平与老年人骨密度的相关性。结果:三组25-(OH)D水平比较,骨密度正常组>低骨密度组>骨质疏松组(P<0.05);三组PTH水平比较,骨密度正常组<低骨密度组<骨质疏松组(P<0.05)。25-(OH)D水平与老年人骨密度呈正相关,PTH水平与老年人骨密度呈负相关(P<0.05)。结论:25-(OH)D水平与老年人骨密度呈正相关,PTH水平与老年人骨密度呈负相关,针对老年人群筛查25-(OH)D、PTH水平有助于早期评估骨密度状态,为临床制定骨质疏松预防方案提供参考依据。Objective:To explore the correlation of 25 hydroxyvitamin D[25(OH)D]and parathyroid hormone(PTH)with bone mineral density in the elderly.Methods:A total of 546 elderly individuals who underwent osteoporosis screening in Jiugong Hospital in Daxing District of Beijing from January 2022 to October 2022 were selected as the study subjects.They all underwent 25(OH)D and PTH tests and bone mineral density tests.According to the results of the bone mineral density tests,they were divided into normal bone mineral density group(n=211,T>-1),low bone mineral density group(n=252,-2.5≤T≤-1)and osteoporosis group(n=83,T<-2.5).The 25(OH)D and PTH levels were compared among three group.The correlation of 25(OH)D and PTH levels with bone mineral density in the elderly was analyzed.Results:The levels of 25-(OH)D in the three groups were compared and ranked as follows:normal bone mineral density group>low bone mineral density group>osteoporosis group(P<0.05).The levels of PTH in the three groups were compared and ranked as follows:normal bone mineral density group<low bone mineral density group<osteoporosis group(P<0.05).The level of 25-(OH)D was positively correlated with bone mineral density in the elderly,and the level of PTH was negatively correlated with bone mineral density in the elderly(P<0.05).Conclusion:The level of 25-(OH)D is positively correlated with bone mineral density in the elderly,and the level of PTH is negatively correlated with the bone mineral density in the elderly.Screening for 25-(OH)D and PTH levels in the elderly population is helpful for early assessment of bone mineral density status and provides reference for clinical formulation of osteoporosis prevention plans.
