动态心电图对心房颤动伴长R-R间期合并房室传导阻滞的诊断价值分析

摘　　要：目的:分析动态心电图对心房颤动伴长R-R间期合并房室传导阻滞的诊断价值。方法:选取2020年2月—2023年2月北京市朝阳区六里屯社区卫生服务中心收治的疑似心房颤动伴长R-R间期合并房室传导阻滞的患者150例作为研究对象,均进行常规心电图和动态心电图检查,比较受试者两种检查结果,包括最快心室率、平均R-R间期以及逸搏、长R-R间期检出情况;以临床综合诊断结果为“金标准”,比较常规心电图检查、动态心电图检查对心房颤动伴长R-R间期合并房室传导阻滞的诊断效能。结果:受检者动态心电图检查最快心室率以及逸搏、长R-R间期检出率高于常规心电图检查,平均R-R间期长于常规心电图检查(P<0.001)。动态心电图检查诊断心房颤动伴长R-R间期合并房室传导阻滞的灵敏度、特异度、准确度、阴性预测值均高于常规心电图检查(P<0.05)。经临床综合诊断,146例患者确诊心房颤动伴长R-R间期合并房室传导阻滞。动态心电图检查诊断心房颤动伴长R-R间期合并房室传导阻滞的阳性预测值比较,无统计学差异(P>0.05)。结论:动态心电图对心房颤动伴长R-R间期合并房室传导阻滞的诊断价值较高,可为临床诊断、治疗及疗效判断提供客观依据。Objective:To analyze the diagnostic value of dynamic electrocardiogram for atrial fibrillation with long R-R interval and atrioventricular block.Methods:A total of 150 patients with suspected atrial fibrillation with long R-R interval and atrioventricular block who were admitted to Beijing Chaoyang District Liulitun Community Health Service Center from February 2020 to February 2023 were selected as the study subjects.All patients underwent conventional electrocardiogram and dynamic electrocardiogram examinations.The results of two examination methods in subjects were compared,including the fastest ventricular rate,mean R-R interval,escape beat,and long R-R interval detection.The diagnostic efficiency of conventional electrocardiogram and dynamic electrocardiogram examinations for atrial fibrillation with long R-R interval and atrioventricular block was compared with the clinical comprehensive diagnosis results as"gold standard".Results:The fastest ventricular rate,escape beat,and long R-R interval detection rates by dynamic electrocardiogram in the subjects were higher than those by conventional electrocardiogram,and the mean R-R interval by dynamic electrocardiogram was longer than that by conventional electrocardiogram(P<0.001).The sensitivity,specificity,accuracy,and negative predictive value of dynamic electrocardiogram in the diagnosis of atrial fibrillation with long R-R interval and atrioventricular block were higher than those of conventional electrocardiogram(P<0.05).Upon clinical comprehensive diagnosis,146 patients were diagnosed with atrial fibrillation with long R-R interval and atrioventricular block.There was no statistical difference in the positive predictive value of dynamic electrocardiogram in the diagnosis of atrial fibrillation with long R-R interval and atrioventricular block(P>0.05).Conclusion:Dynamic electrocardiogram has a high diagnostic value for atrial fibrillation with long R-R interval and atrioventricular block,and can provide an objective basis for clinical diagnosis,treatment an
