摘  要:川崎病是一种主要影响儿童的急性自限性全身性血管炎,以发热、皮疹、结膜充血、口腔黏膜改变、手足红肿和颈部淋巴结肿大为特征,其中冠状动脉最常受累。自1967年首次报道以来,川崎病的冠状动脉并发症已成为发达国家儿童获得性心脏病的主要原因。外泌体是一种由细胞分泌的直径为40~160 nm(平均约为100nm)、具有脂质双分子层膜结构的细胞外囊泡,含有蛋白质、脂质、DNA、miRNA等多种分子,在细胞间通信、免疫调节和疾病发展中扮演着重要角色。近年来,外泌体在多种疾病的诊断和治疗中显示出巨大潜力,特别是在心血管疾病中。本文就外泌体在川崎病诊治中的研究进展进行综述。Kawasaki disease is an acute,self-limited systemic vasculitis predominantly affecting children,characterized by fever,rash,bulbar conjunctival injection,oral mucosal changes,erythema of the hands and feet,and cervical lymphadenopathy,with the coronary arteries most commonly involved.Since its initial description in 1967,the coronary complications of Kawasaki disease have emerged as a major cause of acquired heart disease in children in developed countries.Exosomes are extracellular vesicles secreted by cells with diameters ranging from 40 to 160 nm(with an average around 100 nm)and a lipid bilayer membrane structure,containing a variety of molecules such as proteins,lipids,DNA,miRNAs and other molecules,which play an important role in intercellular communication,immune modulation,and the pathogenesis of various diseases.In recent years,the potential of exosomes in diagnosing and treating a spectrum of diseases,particularly cardiovascular diseases,has been increasingly recognized.This paper aims to review the current research on the role of exosomes in the diagnosis and management of Kawasaki disease.
