经直肠超声引导下前列腺穿刺活检术后急性尿潴留的危险因素分析

摘　　要：目的探讨经直肠超声(TRUS)引导下前列腺穿刺活检术后急性尿潴留的危险因素,为降低前列腺穿刺活检术后急性尿潴留发生率提供参考依据。方法回顾性收集2020年8月至2023年2月于我院进行TRUS引导下前列腺穿刺活检术的331例患者的临床资料,统计穿刺术后急性尿潴留发生率,并分析相关影响因素。结果331例患者中有25例(7.55%)穿刺活检术后出现急性尿潴留。两组的年龄、饮酒、糖尿病、前列腺体积、低蛋白血症、尿道感染情况比较,差异具有统计学意义(P<0.05);两组的体质量指数(BMI)、抽烟、高血压、前列腺特异性抗原(PSA)、前列腺癌情况比较,差异无统计学意义(P>0.05)。多因素Logistic回归分析结果显示,饮酒、低蛋白血症、尿道感染、前列腺体积是前列腺穿刺活检术后急性尿潴留的独立危险因素(P<0.05)。结论饮酒、低蛋白血症、尿道感染、前列腺体积是TRUS引导下前列腺穿刺活检术后发生急性尿潴留的独立危险因素。术前应分析是否存在相关危险因素并采取预防措施,从而减少并发症的发生,改善患者预后。Objective To investigate the risk factors of acute urinary retention after transrectal ultrasound(TRUS)guided prostate needle biopsy,and to provide reference for reducing the incidence of acute urinary retention after prostate needle biopsy.Methods The clinical data of 331 patients who underwent TRUS guided prostate needle biopsy in our hospital from August 2020 to February 2023 were retrospectively collected.The incidence of acute urinary retention after needle biopsy was counted,and the related influencing factors were analyzed.Results Among the 331 patients,25 cases(7.55%)had acute urinary retention after needle biopsy.There were significant differences in age,drinking,diabetes mellitus,prostate volume,hypoproteinemia and urinary tract infection between the two groups(P<0.05);there were no statistically significant differences in body mass index(BMI),smoking,hypertension,prostate specific antigen(PSA),and prostate cancer between the two groups(P>0.05).Multivariate Logistic regression analysis results showed that drinking,hypoproteinemia,urinary tract infection and prostate volume were independent risk factors for acute urinary retention after prostate needle biopsy(P<0.05).Conclusion Drinking,hypoproteinemia,urinary tract infection and prostate volume are independent risk factors for acute urinary retention after TRUS guided prostate needle biopsy.Preoperative risk factors should be analyzed and preventive measures should be taken to reduce the occurrence of complications and improve the prognosis of patients.
