摘  要:目的探讨妇产科变形杆菌混合G+球菌(葡萄球菌)感染的菌种分离方法,并对其耐药性进行分析。方法选取我院妇产科就诊患者58例的变形杆菌混合葡萄球菌共同生长的标本,探讨氨曲南、头孢西丁药物培养基及麦康凯培养基(胆盐)分离培养出的葡萄球菌阳性率,并对其进行药敏试验。结果加入氨曲南药物培养基分离出葡萄球菌比率为65.52%(38/58);加入头孢西丁药物培养基分离出葡萄球菌比率为81.03%(47/58);加入胆盐的麦康凯培养基分离出葡萄球菌比率为84.48%(49/58)。加入氨曲南、头孢西丁药物培养基分离出葡萄球菌比率为77.59%(45/58);加入氨曲南药物、麦康凯培养基(胆盐)分离出葡萄球菌比率为84.48%(49/58);加入头孢西丁药物、麦康凯培养基(胆盐)分离出葡萄球菌比率为93.10%(54/58);加入氨曲南、头孢西丁药物及麦康凯培养基(胆盐)分离出葡萄球菌比率为100.00%(58/58)。分离出的57株金黄色葡萄球菌中,耐甲氧西林金黄色葡萄球菌(MRSA)32株,甲氧西林敏感金黄色葡萄球菌(MSSA)25株,红霉素诱导克林霉素耐药试验即D-试验阳性占22.81%(13/57),红霉素敏感和克林霉素敏感占28.07%(16/57),红霉素耐药和克林霉素耐药占19.30%(11/57),红霉素耐药和克林霉素敏感占31.58%(18/57)。结论氨曲南、头孢西丁药物培养基和胆盐麦康凯平板法结合可在变形杆菌混合葡萄球菌感染性阴道炎中提高耐药葡萄球菌检出率,且做红霉素诱导克林霉素药敏实验,有助于指导临床合理使用抗生素。Objective To explore the separation method of Proteus mixed G+cocci(Staphylococcus)infection in gynecology and obstetrics department,and to analyze its drug resistance.Methods A total of 58 cases of Proteus mixed Staphylococcus co-growth specimens were selected from the gynecology and obstetrics department of our hospital.The positive rate of Staphylococcus isolated and cultured by aztreonam,cefoxitin drugs medium and McConkey medium(bile salt)was discussed,and the drug sensitivity test was carried out.Results The ratio of Staphylococcus isolated by adding aztreonam drug medium was 65.52%(38/58);the rate of Staphylococcus isolated by cefoxitin drug medium was 81.03%(47/58);the rate of Staphylococcus isolated from McConkey medium with bile salts was 84.48%(49/58).The ratio of Staphylococcus isolated by adding aztreonam and cefoxitin drug medium was 77.59%(45/58);the ratio of Staphylococcus isolated by adding aztreonam drug and MacConkey medium(bile salt)was 84.48%(49/58);the ratio of Staphylococcus isolated by cefoxitin drug and McConkey medium(bile salt)was 93.10%(54/58);the ratio of Staphylococcus isolated by adding aztreonam,cefoxitin drugs and McConkey medium(bile salt)was 100.00%(58/58).Among the 57 strains of Staphylococcus aureus isolated,32 strains were methicillin-resistant Staphylococcus aureus(MRSA)and 25 strains were methicillin-susceptible Staphylococcus aureu(MSSA),22.81%(13/57)were positive in erythromycin-induced dlindamycin resistance(D-test),28.07%(16/57)were sensitive to erythromycin and clindamycin,19.30%(11/57)were resistant to erythromycin and clindamycin,and 31.58%(18/57)were erythromycin resistant and clindamycin sensitive.Conclusion The combination of aztreonam,cefoxitin drugs medium and McConkey plate method with bile salts can improve the detection rate of drug-resistant Staphylococcus in Proteus mixed Staphylococcus infectious vaginitis,and the erythromycin-induced clindamycin drug sensitivity test is helpful to guide the rational use of antibiotics in clinic.
