助产士主导照护模式对妊娠高血压疾病患者心理状态、分娩方式及满意度的影响

摘　　要：目的探讨助产士主导照护模式对妊娠高血压疾病(PIH)患者心理状态、分娩结局及满意度的影响。方法选取2021年5月至2023年6月本院收治的80例PIH患者,随机将其分为常规组和研究组,每组40例。常规组采用常规护理模式,研究组在常规组护理基础上采用助产士主导照护模式。比较两组的心理状态、血压、血压达标时间、分娩方式及满意度。结果护理前,两组的焦虑自评量表(SAS)、抑郁自评量表(SDS)评分比较,差异无统计学意义(P>0.05);护理后,研究组的SAS、SDS评分低于常规组,差异具有统计学意义(P<0.05)。护理前,两组的舒张压、收缩压比较,差异无统计学意义(P>0.05);护理后,两组的舒张压、收缩压均降低,且研究组低于常规组,差异具有统计学意义(P<0.05)。研究组的舒张压、收缩压达标时间均短于常规组,差异具有统计学意义(P<0.05)。研究组的自然分娩率高于常规组,剖宫产率低于常规组,差异具有统计学意义(P<0.05)。研究组的满意度高于常规组,差异具有统计学意义(P<0.05)。结论助产士主导照护模式可改善PIH患者的心理状态及血压,降低剖宫产率,提高满意度,值得在临床上推广及应用。Objective To explore the effects of midwife-led care model on psychological status,delivery mode and satisfaction of patients with pregnancy-induced hypertension(PIH).Methods A total of 80 patients with PIH admitted in our hospital from May 2021 to June 2023 were randomly divided into routine group and study group,with 40 cases in each group.The routine group adopted the routine nursing model,and the study group adopted the midwife-led care model on the basis of the routine group nursing.The psychological state,blood pressure,blood pressure compliance time,delivery mode and satisfaction were compared between the two groups.Results Before nursing,there were no significant differences in Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)scores between the two groups(P>0.05);after nursing,the scores of SAS and SDS in the study group were lower than those in the routine group,and the differences were statistically significant(P<0.05).Before nursing,there were no significant differences in diastolic blood pressure and systolic blood pressure between the two groups(P>0.05);after nursing,the diastolic blood pressure and systolic blood pressure of the two groups decreased,and those in the study group were lower than the routine group,the differences were statistically significant(P<0.05).The diastolic blood pressure and systolic blood pressure compliance time of the study group were shorter than those of the routine group,and the differences were statistically significant(P<0.05).The natural delivery rate of the study group was higher than that of the routine group,and the cesarean section rate was lower than that of the routine group,the differences were statistically significant(P<0.05).The satisfaction of the study group was higher than that of the routine group,and the difference was statistically significant(P<0.05).Conclusion The midwife-led care model can improve the psychological state and blood pressure of PIH patients,reduce the rate of cesarean section,and enhance satisfaction.It is worthy of
